Many people have some sort of disability on the Isle. Physical, mental, developmental, and learning, but most go undiagnosed due to the lack of a medical system.

Some are caused by gang violence, and other violence on the Isle. Others are born with their disabilities. Many die, from the lack of medical care, or just wonder what's wrong with them. For those who are born with their disabilities, the causes are usually unknown.

Claudine Frollo suffers from seizures. She just drops to the ground and shakes for a couple of minutes before regaining consciousness. Her father says that the devil has possessed her and regularly does exorcisms to try to release the devil from her body. And Christine Frollo has a club foot. Frollo was so ashamed about having a deformed child, that he doesn't claim her and made Claudine raise her.

The Smee twins, Squeaky and Squirmy, were complaining about their vision for as long as they could talk. So their dad would try to find old glasses in the barge that may help their vision. After a long trial and error process, they finally found glasses that fit, and help their vision.

Clark, the son of a pirate, fell off a roof, and somehow lived. But he became paralyzed. He couldn't walk and had trouble moving his arms. He needed help doing most things, he couldn't leave the ship anymore because the Isle doesn't have mobility aids. Then Gianna, the daughter of a pirate, became an amputee after having her hand cut off during a fight. She lived for three days before the blood loss killed her.

Charlotte, the daughter of Gaston, is deaf. She can't hear anything. Her sister Laci tried to help her communicate but it became too hard. Eventually, they found a book with the alphabet in sign language and they started signing. After a couple of years, with the help of Harriet Hook, they created Isle Sign Language so everyone could communicate. Not everyone knows ISL, but some do.

Then there's Stephanie, the daughter of Jack, one of Clayton's men. She was allergic to shellfish, but due to the lack of medical care on the Isle, no one ever knew. So one day she collapsed in Ursula's Fish and Chips, and no matter what anyone did, she still wasn't breathing. So Uma declared her dead and returned her body to her father, who didn't care.

Being disabled on the Isle is hard, most people don't even know if they have a disability. Many people die daily or suffer because of their disability, and there's nothing anyone on the Isle can do about it.
