This is a somewhat old story, which I edited and added a few more things, and decided to republish. It's a nice IchiRuki, with slight hints of Kaien x Rukia. It is not romance, it is something more gloomy and dark. It has a happy ending, within everything that happens.

Chapter 1: Little Bunny
— Miss Rukia Kuchiki, you will be transferred to the Rukongai Mental Care Facility to serve your sentence.
It was what that bald man with a huge white beard and a black robe said in that trial of The State against Rukia Kuchiki, because none of the victims had survived.
Rukia didn't speak at that trial other than when they asked her questions, and even when she did answer them, she did so in an erratic and strange manner. She also did not try to resist at any time being taken or brought by the police officers who were in charge of her not fleeing the place, although it was practically impossible because her legs were held in a pair of shackles that made it difficult for her to walk.
Rukia was so dangerous that they didn't want to risk her taking advantage of their carelessness and running away.
Rukia had been arrested after she was found at a crime scene that was listed by the news as "the most macabre and bloody of the last decade". And the news was not wrong because Rukia murdered an entire family in one night.
The bodies that were found belonged to two adults, a man and a woman, and two small children between 7 and 9 years old.
Rukia had dismembered them and scattered the remains all over the place in a strange and macabre work of art that consumed all of her attention. If Rukia hadn't stopped to play with the girl's hand, with which she painted the walls with blood to finish her tribute to the demons with unpronounceable names, they wouldn't have caught her on the spot.
Her defense attorney, who was a guy with strange orange hair and who had an incredible fame because he managed to defend the indefensible, presented evidence that supported the fact that Rukia didn't have a mental health that was considered "fit" for the social environment; along with some other problem that they obtained as a diagnosis from the different psychologists who evaluated her, including her primary psychologist.
Rukia had reduced her visits to the therapist because she had shown "considerable improvement" in her mental and emotional health.
Rukia wasn't dumb, in fact she was incredibly smart, and as soon as the Evil's Lawyer explained to her what they were going to do, she stuck to the plan. So she pleaded not guilty, the lawyer of evil presented evidence that she had a mental health problem, they bought everything they could buy, and she ended up in a mental health clinic.
The sentence was for several years but it was the best option for everyone. Rukia would not spend her whole life there, since the lawyer of evil would be in charge of doing everything possible for her to get out of that place by one means or another, but as long as she was there she would be cared for as befitted one of her status.
Rukia said goodbye to her father and brother with tears in her eyes when the officers took her out of the courtroom and it was a remote goodbye because, for safety reasons, her family was not allowed near her.
If her father had known her well, then he would have noticed that those tears were as fake as a three dollar bill.
Rukia felt everything at that moment, except pain and suffering. She was really upset because it wasn't the first time she had done it, killing someone or eating someone, but it was the first time she had been caught.
The road to the Mental Care Center seemed to take forever, so much so that she began to talk in silence with her inner voice; the one that appeared after her uncle decided that she was the perfect tribute for those strange rituals where strange demons with names difficult to pronounce were worshiped.
The officers with her were on the lookout for any signs of aggression on her part, though Rukia was just getting a scolding for being careless and not running away when she could.
"Little bunny…"
The voice in her head spoke, using that nickname that the lawyer of evil had given her. Rukia liked it when the evil's lawyer called her that.
"We're in trouble but I'll hide our hunger so they'll let us out of here faster."
And then that voice disappeared.
Rukia mentally cursed the voice in her head for leaving her alone at that moment, but she couldn't do anything because that voice would sometimes disappear for short periods of time, usually when she was really happy and calm. When she was in trouble, with a lot of anxiety or her mind became chaotic from memories of the past, that voice appeared and took her for a walk down that avenue of depravity and death.
That voice stayed with her and gave her the courage to taste the world in a way that only that voice had allowed her to do. Without that voice, Rukia might not have done much of what she did in her life.
When they arrived at the place, she was taken to a room in an area that should be destined for dangerous criminals like her, because they had to pass at least three security filters and several other things.
Rukia saw everything and the only thing she saw was too much white. She didn't understand why all medical sites should have white all over them, when red and black were prettier.
— If I take your straitjacket off, will you promise not to move and stay quiet, pretty girl? — Asked the male nurse, who led her to that room before releasing the straitjacket ties that kept her suppressed.
— Quiet? Why? Do you like it with quiet girls? I promise I'll pretend I'm dead. — Rukia said that as if it didn't matter much. — I won't move at all. Just don't cum inside, only one is authorized to do it inside.
The male nurse, who took her to that place just laughed at that, took a syringe with a clear liquid and injected it into Rukia's thigh. Rukia felt that liquid begin to take effect very quickly and she felt dizzy, the guy held her so she wouldn't fall and Rukia didn't know what else happened because everything turned dark and cold.
When she regained consciousness and opened her eyes, she discovered that she was in a white room with leather ties on her wrists and ankles, and no matter how hard she tried to free herself from them, it was impossible. She only stopped trying to free herself when a tall, black-haired man entered the room causing her to focus all her attention on him.
The man was wearing a medical gown and Rukia immediately looked around for the doctor's name embroidered on the fabric, but she couldn't find it anywhere and that made her knit her eyebrows together in a gesture of anger that was too beautiful and dangerous.
— Rukia Kuchiki, I'm Kaien Shiba. I will be your doctor.
Rukia smiled seeing her doctor with that complicity that came from being with someone well known.
— Little Snowflake, the feasts are in private and not with the family. — Kaien said familiarly without checking the papers in his hand. — Plus it was a waste of meat. You could have worn it on your turn to be the hostess.
— He was a pending account that I had, his meat would not have served. Is this where you get the meat from? — Rukia asked as Kaien untied the ties around her ankles and caressed her leg cheekily.
— It could be… do you want to play or do we go directly to what interests us? — He asked looking into her eyes.
— We'll have plenty of time, we can play around a bit before you explain to me how this is going to work. Ichigo promised me there would be no medication.
Kaien took off his medical gown and set it aside before removing the ties from Rukia's wrists.
— I'll send some special ones for you. We don't want your beautiful mind to be damaged, but you must behave yourself and not cause any trouble for the nurses.
Rukia smiled at those words and rubbed her wrists which were sore. Kaien wasn't allowed to tie her to the bed, only Ichigo could.
