Judging by the tone, I'd say these were given to the other three hobbits in the Fellowship from Frodo shortly before he departed for the Grey Havens. Very sweet indeed.
Humbled
To Samwise the Brave. What more can I say?

11 years old is Samwise
small and blustery.
Tripping over words and feet,
Digging in the earth with brown hands,
And always so easily
humbled
21 years old is Samwise,
yet wise as the Elves.
Selfless in all he does,
A hero, more than I could imagine.
Yet he is still so easily
Humbled.
54 years old am I,
aged and worn.
But weariness is forgotten, Samwise,
When I think of what you've done for me.
And it's all I can do
but be humbled.
Many winters old is Samwise,
Grey and alight.
Gentle mists hitting your face
As you go sailing, sailing o'er the sea.
And it's all I can do
But be humbled by you,
My Samwise, The Brave.
