Itachi had torn apart everything that was every important to Sasuke. He had destroyed his family; had destroyed his life. Itachi's betrayal had ripped apart his heart.
So this is only fitting, Sasuke thought to himself as his brother roughly entered him. This is the only things he's ever done to me, he thought as his brother tore him apart from the inside.
He just keeps tearing me apart.

Sometimes, when Sasuke was feeling particularly masochistic, he would try and pinpoint the most painful part of the massacre of his clan. Was it the fact that his parents were murdered in cold blood? Maybe it was the loneliness he had suffered since that night. Was it the fact that he now had to betray both friend and village in order to fulfill his role as avenger? Occasionally he wondered if the betrayal of his brother hurt the most. The fact that the person who had been the object of his love and respect for so many years could wound him so deeply with no regret. Sasuke knew that he had loved his brother; he was never masochistic enough to admit that he still did. He would never stop loving him. And that's what hurt the most.

Kisame had asked him once why he had spared his brother. He gave him the same answer he gave himself: to make things interesting. Knowing that someone was out there whose sole desire in life was to kill him gave him a rush of satisfaction. Kisame had rolled his eyes and muttered something akin to 'sick bastard' under his breath. Itachi shrugged mentally; he would kill his little brother eventually, why rush it? The fun was in watching the boy (Sasuke would always be a boy in Itachi's eyes) try and continuously fail to kill him. If there had ever been another reason for him keeping his brother alive, Itachi had forgotten it long ago.
