Just because it refers to cutting does not mean that she actually cuts herself! If you really need to know the story behind the poem message me and I will explain! But it is not meant as Hermione is a cutter, she's just having a really bad day, and this is sort of the result of that. Sometimes the people you least expect are the ones that come up with the most depressing things.
