The Gaang would be shocked, but mostly okay with Zuko dating more than one girl, and with said girls also dating each other. They'd be plenty of jokes about his confusing taste in women since he is dating both Mai and Ty Lee. Ozai would be absolutely livid that Zuko managed to get not one, but three girls to date him. He can't understand why Azula of all people would like him.
Ursa and her husband would be confused by it since it is very unusual, be absolutely appalled by the incest, and wonder how the hell they're going to explain that to their daughter - Azula told her for them, and the kid was confused but supportive. Same goes for Tom-Tom. Ukano and Michi would just be glad that their family is marrying into royalty. I don't know enough about Ty Lee's family to picture their reaction.
Iroh would be proud of his "son" for getting three girls… then have a nervous breakdown once he realized one of said girls was Azula. Pakku is Pakku, so he'd find a way to be an asshole about it. Piandao and Jeong-Jeong don't love the idea, especially of the incest, but they've seen weirder shit. Bumi wants to officiate the wedding.
The other world leaders would act appalled, but be secretly glad that there's nothing they could possibly do that'd be as controversial and fuel for gossip as that. They'd all probably want to invited to the wedding because it'd be epic.
The general public in every nation would react as all real-life normal people would react if a world leader was marrying three different women - one who he happened to be related to - and said women were also marrying each other. There'd be lots of jokes, gossips, false moralism, degenerates being horny, and no one would shut up about it for a single second.
