They'd probably start as just Zuko's mistresses, because Azula would still be mad at them and Mai would still resent her. But, since they'd end up seeing each other A LOT, they'd slowly get over their issues, with Zuko and Ty Lee acting as mediators. The first time they all have a foursome, they just know that they're now Azula's mistresses too.
I feel like they'd all treat each other pretty equally, but Azula would 100% use the fact that she is technically the "official" wife for some powerplay in bed, and Zuko would loooove being more dominant with all of them since he's the Fire Lord. When he thinks Azula is being too bratty or selfish, he allows Mai and Ty Lee to do whatever they like to her, which often ends with Azula bruised, exhausted, and too overstimulated to do anything.
