Ty Lee is the one who starts it. She'd probably start by just nuzzling Azula and Mai during a sleepover, and cuddling with them until they're all asleep to show affection, like she saw some animal at the zoo doing with their babies. It'd become the new normal for them when they're alone together, and it'd slowly escalate to licking, biting, and dry humping, because now Ty Lee is imitating how she saw animals showing affection to their mates. Obviously she'd add in some kissing, which would start innocent, but turn into make out sessions.
One night however, when the girls are all in their own homes, Azula is lonely and needy, and goes to Zuko's room, saying she wants to play a game. She'd start off with just the more "innocent" bits of what she does with her friends so Zuko would lower his guard, but she'd manage to do everything she wanted that night. The next day, Zuko is blushing furiously whenever he looks at her… and then goes to her room at night because he wants to do it again.
Mai and Ty Lee would start staying over a lot more after that (and Zuko would have been so embarrassed but so happy the first night the four of them "played" together), and the situation would keep escalating - now instead of just dry humping, they'd probably be masturbating together or helping one another masturbate, and it'd eventually lead to some oral, and then finally, full intercourse. By that point they're all very aware that this is inappropriate, but they don't care, it just feels too good.
