Cause of Injury: Plasma Grenade Hot Potato
Abstract:
The incident occurred while several XCOM personnel were engaged in a game referred to as 'Plasma Grenade Hot Potato'. According to those present, a mistimed throw of the plasma grenade resulted in the instantaneous liquidation of the patient's hands and forearms. Additionally, severe burns were identified on the outside of the patient's thighs and top of their shins. Cauterisation of the wounds due to the extreme temperature combined with the swift application of first aid mitigated the immediate risks due to blood loss and infection.
The patient has consented to being transferred to the MEC augmentation project and, beyond the required amputations, is expected to make a full recovery.
Addendum: Some senior personnel have expressed concern about providing such, unpredictable, individuals with access to rail guns.

Cause of Death: Death by Floater (Deceased)
Abstract:
The incident occurred while unloading the Skyranger following successful completion of strike team operations (Refer to XCOM-AAR-21D91 for further details). While unloading a Floater specimen (Deceased) the patient identified that poking the specimen in a specific point between the mechanical augmentation and the organic body induced a spontaneous arm movement in the specimen. This was considered an amusing act of post-mortem spasm. Despite objections from other members of the ground crew the patient persisted.
It is believed the patient subsequently struck an exposed circuit which resulted in an unintended discharge of the Floaters boost pack. This discharge caused both the specimen and the patient to be propelled at high speed into the hanger wall. Both the specimen and the patient did not survive intact. The medical technicians have so far had significant difficulty separating the specimens for further analysis.

Cause of Death: Exsanguination, Multiple Organ Failure, Severe Tissue Damage (Threshing Machine)
Abstract:
The incident occurred while investigating reports of alien activity in a farm in rural Kansas, US (Refer to XCOM-AAR-79J9 for further details). The patient looked to gain a better vantage point and climbed a ladder onto the roof of a barn. While crossing the roof an unstable beam gave way causing the patient to fall through the roof.
During the fall the patient struck the outer body of the threshing machine parked inside the barn, inadvertently activating the machine, before falling to the floor. Statements from the other members of the strike team confirmed that the patient survived the fall with only superficial injuries. It was while being distracted at the prospect of having survived the fall unharmed that they stumbled and fell into the now rotating blades of the machine.
Attempts at resuscitation were deemed unnecessary by the strike members present. Further autopsy has proved difficult due to the limited sample that could be returned for analysis.

Cause of Injury: Shot self in foot
Abstract:
Patient accidentally discharged a laser pistol into their foot while attempting to recreate the cowboy sharpshooter manoeuvre (Shooting from the hip).
A Magnetic Resonance Imaging (MRI) scan of the patient's brain was performed at the request of Dr Albright, stating that such a level of stupidity could be symptomatic of deep neurological damage. Further diagnosis to be performed pending results of the MRI.
Addendum: Training Captain Alicia Monroe has recommended additional weapon handling training for all strike team and security personnel of Corporal rank or lower. Additionally, Rookies are not to be issued with anything sharper than a spoon (And only with supervision) until training is complete.

Cause of Injury: Electrocution, Fall
Abstract:
The incident occurred while performing maintenance on the Skyranger. The patient did not follow proper maintenance procedures (Refer to XCOM-MAT-81G9 for further details) to ensure that power was removed prior to maintenance of the electrical system.
While using a metal rimmed mirror to inspect the area the metal casing touched a live circuit. The resulting electrical shock resulted in an intense muscle spasm significant enough to cause them to be propelled out of the Skyranger before falling an estimated five feet onto hanger floor.
The patient was brought to the infirmary with assistance from other ground staff. The Junior Technician who was first responder to the incident described the patient's expression as 'Shocked'.

Abstract:
The patient reported to the infirmary for their monthly physical examination. After requesting a urine sample the patient returned with a sample cup that was glowing bright yellow and asked 'Is this normal?'.
The patient was questioned if they had ingested anything unusual. The patient affirmed to having drunk liquid Elerium because 'My grandad was in the Navy and used to drink torpedo fuel, never did him any harm'.
Subsequent tests identified that the patient had actually ingested fluorescent food colouring prior to their exam.

Cause of Injury: Traffic Collision
Abstract:
The XCOM Training Department commissioned an instructional video for new XCOM Strike Team Members. This required a member of the strike team (The patient) to dress as a Muton as part of the demonstration.
The patient left the film set in full costume immediately following the days filming and drove home on leave to attend their daughter's birthday party. This resulted in numerous reports to the local police of witnesses seeing an alien invader speeding and running red lights.
The patient reported significant difficulty in operating the gear stick with the costumes large, gloved hands. A moment of inattention caused them to run over a pothole and, in attempting to regain control, struck a lamppost.
Although uninjured in the impact the car did not restart. This led to further reports to police of an alien fleeing a hit and run as the patient continued on foot.
Upon returning to XCOM the patient was treated for minor abrasions and cleared of any concussion or other serious injuries. The matter has since been transferred to the XCOM Public Relations department.

Cause of Death: Death by Misadventure
Abstract:
The incident occurred while responding to an ADVENT attack on the Seven Rivers Resistance Outpost (Refer to XCOM-AAR-71E0 for further details). Patient sustained injury to the leg after falling from a height onto a piece of sharp metal.
Patient was taken to the Resistance "Doctor" who insisted that, as gangrene was already setting in (A diagnosis contested by the autopsy), emergency attention was required to save the affected leg. The "Doctor" began his examination by listening to the patient's heartbeat with the use of an upturned shot glass. It had to be pointed out by another member of the strike team that the patient's heart was located on the other side of the chest.
The "Doctor" then attempted to cauterise the affected area through use of a trowel and a blowtorch. When this proved ineffective the "Doctor" made the determination to amputate the affected leg. They then proceeded to amputate the wrong leg. The resulting blood loss and shock, inevitably, resulted in death.
Addendum: Disregard cause of death, that's just Dr Albright being sarcastic.
