They both knew. They knew she was sleeping with them both.
John knew Elizabeth was sleeping with Rodney, but John didn't care. Most men would have ended it on finding out their girlfriend was sleeping with another man, but then most men didn't want to sleep with that man too. John would never have a chance with Rodney McKay. So, he continued his relationship with Elizabeth, thinking that if she knew that he knew she would end it. John kept out of the way when she was seeing Rodney, he never came home early or surprised her, because if he did he might catch them together and his perfect arrangement would be over.
Rodney knew Elizabeth was sleeping with John, but Rodney didn't mind. Finding out that his girlfriend was sleeping with another man was a surprise, but then he wanted to sleep with that man too, so he never said anything. He'd never have a chance with John Sheppard. Rodney worked out how to make sure he never caught John and Elizabeth together, he never questioned her on where she had been or why she was late and he made sure never to put her in a position where she might admit to the affair. After she had been with him, he knew, but he never said a thing, because if she found out he knew it'd be over.
Weeks past, and they continued on as normal, as normal as they could. The two men made sure they never uncovered Elizabeth's unfaithfulness, and Elizabeth never found out they knew. The fact that they were using her crossed their minds frequently; but they thought that this was the only way. And, for the moment, they tried not to let that bother them, because as far as they were concerned, this would be the closest they would get to have each other.
So, when Elizabeth found out they both knew, she wondered why they never stopped it. She noticed the fact that she had never had any 'close calls', and neither John nor Rodney ever questioned where she was during her absences. Then one day as she watched them together she realised the reason why they let the affair continue and why they let her sleep with them both. She realised that John and Rodney wanted to be together. And, although she was hurting, she let herself be used by them, so that in some strange way, John and Rodney could be together.
