Izuku's POV

"Thank you!" Waving goodbye to the cart driver, Izuku Midoriya leapt off the side and landed on the pavement.

Well, this was it! After three years of studying he was finally a doctor! He looked over the white band on his wrist appreciatively. It was made of fine waterproof cloth, perfect for someone who might have to deal with all manners of bodily fluids.

Moving to a new city was an honestly tough decision. Leaving behind his mother, all his friends and his hometown hadn't been easy, but the practical part of his mind knew it was the right decision, their hometown was too small to support a doctor of his skills. Not to mention, he could escape the shadow of his father that way.

He had even managed to get a brand new space. It wasn't easy, he had had to use practically all his connections, but he had got it. It had been constructed years ago before the war, and went unused after the city's economy shrunk and its population fled. It wasn't technically brand new, but it was untouched and that was enough.

He was snapped out of his thoughts by someone vomiting on his shoes.

It was a girl, roughly his age. Judging from her human-like appearance and the pink dots on her cheeks she was a Sky Fairy, a monster that used an internal organ to float weightlessly despite lacking any external means of flight or propulsion. They were alike to humans in almost any way, appearance, weight, biological needs, the pink dots and the flying organs being the only difference. The especially powerful ones could even transfer their weightlessness to others, through magic that could be learned, although she still seemed a bit young for that. In other circumstances he would have called her cute, even pretty, but not in her current state.

"Oh, oh no!" Even though she looked to be on the verge of passing out the girl looked mortified.

"I'm so sorry-"

"I'm fine." He leaned forward, the part of his mind that would have been flustered or disgusted taken over by the part in charge of professionalism, seeing how green she looked and hearing the weakness in her voice. "Are you ok? You don't look so good."

"I-" she seemed to notice his white wristband for the first time. "Are you a doctor?"

"Yep!"

"You're not going to make me pay for this right?"

He looked at her, shocked. "For some advice? Of course not!"

She swallowed, evidently fighting off another wave of nausea. "I've been nauseous, and my powers have been turning on and off on their own." As if to prove her point she started floating again, and he grabbed her again and dragged her down.

He frowned. "How long has this been going on?"

"A few weeks, maybe a month?"

A month?! He was surprised enough to let her go, and she started to float off before he hurriedly managed to grab on to her foot. How could- how could any Sky Fairy go that long with their flying organ acting like this? Flying organs were as vital as human kidneys, with even more obvious effects if they were unhealthy.

"I'm taking you to my clinic."

"Wait what? But I can't-eek!"

He grabbed her hand, causing her to squeak for some reason, and dragged her to the building.

Ochako's POV

Ochako had been warned for years by her parents about scams. Not having much money to begin with, and with the city still having a high crime rate from the effects of the war, she had to be alert for any criminals.

One scam they had warned her about repeatedly was the fake doctor one. It involved some "doctor" pulling her aside on the streets, telling her she had some illness then forcing overpriced bottles of "medicine", actually water, on her. In her eyes, this was by far the worst one, since it preyed on the weak and sick who couldn't afford doctors and had to rely on streetside medical vendors.

But this didn't seem like a scam. She was the one that puked on him after all. Her face burned when she thought back to her vomiting on his shoes. What was she doing? There was a drain right beside him!

She sneaked a peak at the boy who was currently dragging her like a balloon through the streets. He was young, probably her age. The most striking feature about him was his eyes and hair, they were almost shining green.

The only question was what was he? A woodland sprite? A human maybe? That seemed unlikely, humans were pretty rare in this part of the world, but it would be rude to ask.

She sighed. Great. The first time she was holding hands with a boy, an attractive one at that, and it was because she had puked on him.

'Are you okay back there?" He looked up at her, snapping her out of her daydream. "It might be kinda embarrassing but this is the only way I can get you there."

"I-its ok! I'm fine!"

'Ok, hang in there." He gave her hand a comforting squeeze.

His hands are so soft...

However, the sense of calmness and ease the doctor had given her once again disappeared when she realised that she would have to pay for whatever treatment the doctor offered her. She could refuse, but she needed the advice and the hopefully cheap meds.

The girl became more and more panicked as they started approaching the middle class districts. She might be able to scrape together enough cash for a lower class doctor but not a middle class one!

To her enormous relief, he stopped right at the edge of the border between the two areas, and turned to face a small building. The building's exterior looked old, built with the pre-war style, although the doors and flooring was new. It was probably newly renovated, with the functional but old parts left alone. It's ok, she could afford this place. Probably.

"Give me a moment." He placed her hand onto a pole, giving her something to hold onto, while he pulled out a bunch of keys. There were chains over the door, with over a dozen locks, and he laboriously unlocked all of them, pulling them off and stacking them in a corner.

Opening the door, he dragged her into a waiting room, motioning for her to grab onto a heavy table. They waited there for a few minutes before the floating finally wore off, and he led her inside.

The internal area was also in the pre-war style, although she could tell it had never been used, judging from the dust on boxes of untouched medical equipment and supplies.

"Sit down over there." the doctor pointed at a chair beside the largest table in the room, and pulled out packets of herbs and powders. She sat down, sending small clouds of dust flying up from the chair.

"It sounds to me that you either have a slight infection, lack of nutrients or an internal wound. The last one is unlikely, since you would have felt some pain by now, so it's probably one of the former two. Open wide."

He shoved a wooden stick into her open mouth, pressing her tongue down. He then checked her pulse, her temperature, and rapidly wrote down something on a piece of paper.

"So, what's your name?"

"Ochako Uraraka."

He grinned at her. "My name is Izuku Midoriya."

"Are you new here?"

"Yep! I just moved into the city. I- Ah, hang on." He leaned down underneath the table to pull some book out, flipping through the pages. He started rapidly muttering to himself, staring at the pages until his eyes brightened as he found what he was looking for.

"Ok, from the looks of it you have some slight malnutrition. Nothing major, just take some supplements daily and you'll be back to normal within a few weeks. Here, I'll write you a prescription."

She smiled at him, waiting until he turned around to grab the herbs before discreetly opening up her coin pouch and fiddling through. Ok, she could pay the consultation fee, and still have a few coins left over for dinner! At least now she knew what was wrong with her.

She looked up, and her blood ran cold when she saw what he was wrapping up in waxed brown paper.

"Here." He pressed the packet of medicine into her hands. She looked down at the surprisingly heavy package, wondering whether it was possible to reject it, or if it was easier to just throw back at him and run away before he could make her pay for it.

"This is Ruby root powder, it'll help with the symptoms. Take it once a day, one teaspoon mixed with one cup of hot water, maybe with a bit of milk if you want to cut the sourness. If you're-"

"No!" She pushed the packet back into his hands.

"No?"

"I can't afford this!" She looked around frantically. "Don't you have Coral root or something?"

"Coral ro- Is that what you've been taking? That doesn't help at all!" He looked shocked, almost offended. "Look, don't worry about the price."

Ochako's eyes widened. "Ex-excuse me?"

He pressed the packet back into her hands. "Don't worry about it, pay me back when you can."

"B-b-but that could be weeks!" It would be more accurate to say months, but the small sense of pride she had didn't want her to embarrass herself any further.

"It's alright, take as much time as you need."

"B-but can you afford it?" Judging from his youth and the looks of his clinic he had just started his career, she was probably his very first patient. He couldn't possibly be giving away something like this for free to a person he just met!

But he leaned forward and grabbed her wrist, enveloping it in his soft hands, and smiled gently at her.

"I don't want to fail the very first patient I ever saw in this city."

Izuku's POV

He had been hoping that what he said was comforting, but apparently not, because as soon as he said that Uraraka immediately slammed her hands over her face and started to float off.

"Ah! Miss Uraraka, careful! The lights!"
