John quickly laid Sam down and stood protectively in front of him, as did Dean and Mac. Joshua closed the last spot in the protection wall for their youngest. While Caleb crawled next to Sammy to protect him with all he could, minding his broken leg.
The Black Annis howled again and charged at the hunters.
"How do you kill the sun of a gun?" Dean shouted as his fast reflexes saved him from being the victim to the monster's long claws.
"Stab it in its brain through its mouth," Mac yelled back as he fired at the Annis to distract it.
Caleb watched the battle unfold from his spot on the ground, hating his broken leg for not being able to join his family.
Just then, the Black Annis took notice of him and sped towards him, claws out, faster than any of the other hunters could react.
He heard shouts of "No! Damien!", "Caleb!", and "Son!" and closed his eyes as he waited for the pain. But it never came. He opened his eyes and was horrified by the sight of a smaller body standing in front of him, then the figure collapsed, blood pooling around him.
Caleb stared horrified at his little brother's bleeding body. Sam had thrown himself in front of him, and taken the hit meant for him. Said little brother was staring at him, tears of pain trickling down his fever hot cheeks, yet he was smiling.
"Sammy…" Caleb whispered. He didn't even register the shouts of fear and horror of the others. Nor did he register the shout of rage of a father that just witnessed his sick child being mauled by the monster, and thus charged at it and effectively killed it. His mind only focused on his little brother, bleeding out in front of him.
A few minutes before:
Sam opened his eyes at the obvious sounds of a battle around him. Groaning at the pain that radiated from his head to his toes, he looked around and dazedly saw his family fighting the Black Annis.
He tried to shake the fog from his head, but failed until he saw the Black Annis change trajectory and lunge with its claws outstretched at Caleb, who was sitting on the floor unable to fight thanks to his broken leg.
It was as if electricity was jolted through his body, causing him to stand up and bolt towards Caleb, throwing himself in front of him.
His abdomen erupted with pain as the Annis's claws ripped through his skin like butter, and he swayed in his place as he heard yells and shouts. Sounds blurred together and he saw his father kill the monster as he fell to the ground.
He found himself staring at Caleb's horrified face, and the teen smiled, blood leaking from the corner of his mouth. He saw Caleb's lips move, but he heard nothing. He didn't need to.
Taking a shuddering breath, Sam gripped Caleb's hand weakly. "It's… okay …," he whispered.
Then everything went black.
Now:
Caleb watched as Sam passed out, then as his family crouched around them after the Black Annis dropped, dead.
Mac started barking orders, to put pressure on that and place a wet cloth on Sam's forehead. But Caleb just registered the blur of voices and colors as the shock of everything and his injury caught up on him. The only thing that he saw before he passed out was Sam's pale, feverish face.

Beep… Beep… Beep
Caleb awoke to a beeping sound and low murmuring. Opening his eyes, he saw the white sheets, white walls and the sharp smell of antiseptic.
'Hospital,' he thought.
"Damien? Can you hear me?" came a voice from his right.
Turning his head, he was met by the worried face of Dean.
"Deuce, what happened?" the psychic winced at how hoarse his voice sounded.
Dean noticed and handed him a cup of crushed ice to which Caleb nodded gratefully as they soothed his sore throat.
Dean sat down with a grim expression. "What do you remember?"
Caleb thought for a moment, brow furrowed. "The Djinn, not a Djinn… Broke my leg and tied us up… then dad and Sa… Oh God SAMMY!" He yelled bolting forward but Dean placed a hand on his chest and pushed him backwards.
"Calm down Damien, Sam's… well he's alive."
Caleb stared at him "What the heck happened, Deuce?"
Dean sighed "After you two passed out, we took you to the hospital. That was around six hours ago. They X-rayed your leg and you had to get minor surgery to set the bone. You'll be fine but you'll have to wear a cast for six weeks."
Caleb shook his head "Deuce, I don't care about what happened to me. Sam, where's Sam?"
Dean pressed his lips together, and Caleb knew that Dean was trying to control his emotions which was why his heart sank as the situation had to be bad. He even tried to read his friend but the meds he was on screwed with his talent and he stopped the try instantly, feeling a sharp pain in his head.
"Sam lost a lot of blood, Damien. That, and the virus he already had took a toll on his body. He almost stopped breathing on the way to the hospital. His heart stopped twice, once in the ER and in surgery. They got him back but he's extremely weak and he's got a long fight ahead of him. The wretch's claws didn't cause any internal damage except for a small tear in his stomach, but as I said he lost a lot of blood. He needed 3 units of blood, thank God they had his blood type because it's not everyday that a hospital we visit has O neg units on standby, especially since it's not a big hospital"
Caleb paled "Where is he now?"
"He got out of surgery an hour ago so he's still in recovery. Dad's with him and Joshua's getting Jim, he just arrived here. I wanted to see the kid but this hospital has some stupid rule that only parents are allowed in the recovery ward," Dean answered angrily.
"I want to see him."
Dean smirked "We knew you were going to say that, so Mac pulled some strings. Once Sammy gets out of recovery, he'll stay here in your ICU room instead of the PICU."
The psychic nodded "His virus?"
"Still there and strong. His fever isn't coming down. That and the post op fever made it worse, and Mac is worried about infection from the wound," Dean said, concern evident in his voice and on his face.
Caleb closed his eyes at the waves of raw emotions pulsating from Dean, he was barely able to filter at the moment. He knew, as a big brother, he had to put a cap on his own emotions to reassure the boy who was a little brother to him.
Forcing a grin, he nudged Dean gently. "Come on Deuce, we all know Sammy's a fighter. He'll be fine."
Dean shot him a look "You really want to have a chick flick moment."
Caleb rolled his eyes "Humor me."
"Fine, you didn't see him, Damien. When his heart stopped…" Dean's voice trailed off and he shuddered as he relived the moment that was one of his worst nightmares. Caleb placed his hand on Dean's shoulder for support.
"But he's okay now, isn't he? His heart is beating again and he's fighting. Of course he will, when he has such handsome brothers to wake up for."
Dean smirked "You mean a handsome brother and two ugly brothers, you and Josh look like overgrown mountain trolls."
Caleb glared at him "Whatever, Deuce. The point is he's still alive and he's fighting. Because that's what we taught him to do."
Dean nodded "Thanks, Damien."
"You're welcome, Deuce. Now if you tell anyone I started a chick flick moment, I'll kill you."

Caleb had seen Sam in the hospital before. Whether it was, surgeries, wounds from hunting, or sicknesses caused by his weaker immune system that landed it in the hospital. But he had never seen him as vulnerable and fragile as now.
Sam was pale, so pale that he could've blended with the white sheets he lay on if not for his hair. Bandages peeked from under his hospital gown and IVs were running in and out of his arms and hands delivering fluids, blood, and medications, as well as countless monitors on his arm and chest. But what bothered Caleb the most was the long tube that protruded from Sam's mouth and was taped to his lower lip.
They had brought Sam to their shared room about ten minutes ago, and Caleb couldn't tear his eyes away from his little brother. They had pushed the two beds together so that Caleb could be close to Sam and hold his hand if he wanted to.
Right now, Dean and Joshua were passed out on their chairs, having fallen asleep after making sure Sam was transported to the room safely. Jim had taken Mac to the cafeteria to make sure The Scholar ate after taking care of both Caleb and Sam since their arrival at the hospital, and John was sitting on the other side of Sam's bed by the window, holding his son's hand tenderly and stroking his hair.
Caleb had never seen John exhibit such tender touches of affection except when one of his sons were injured, and that extended not only to Sam and Dean, but Caleb and Joshua as well.
"Don't worry, John. He'll come back to us," Caleb assured the man who was a surrogate father to him just as Mac was a surrogate father to Sam and Dean.
John smiled softly, never tearing his eyes away from his son "I know, he always does," he replied in the softest voice Caleb had ever heard from him, and the psychic had to fight the bulge in his throat as he felt the rush of love and pride coming from the father towards his baby.
A few minutes passed by with Caleb and John holding Sam's hands and staring at his face until Caleb broke the silence.
"When can he get rid of the breathing tube?"
John pressed his lips together, he hated the darned tube too. "Mac said he'll try to wean him off today. His lungs weren't hit thankfully but with the trauma his body went through and the anesthesia from surgery, he wasn't going to take any chances."
Caleb nodded then winced slightly as he shifted and a pang of pain went up his casted leg.
John's head whipped up and studied Caleb's face, then pressed the button on Sam's bed. A nurse appeared after a minute.
"Hey there, what can I do for you?" She asked with a kind smile.
"Caleb is experiencing some pain in his leg, could you give him something for it?"
Caleb's eyes widened "I don't …" he trailed off as he saw the dangerous glare that John sent his way. Sighing, he relented "Fine you got me."
"Of course, I'll give you something right away." The nurse then headed to the locked cabinets and produced a vial and a syringe. Drawing up the right amount, she injected it into Caleb's IV and left the room.
"Get some rest, Caleb." John told him softly and watched as Caleb's eyes slipped close and his breathing got even some minutes later.
