Failure Analysis

In this Deep Space Nine novelette, a catastrophe thought to be impossible has occurred; Sisko, Dax and O'Brien are given the job of trying to find out what happened. The story is a mystery; not so much a "whodunit," but rather a "HOWdunit" and a "WHYdunit." You won't be left cliff-hanging; the solution to the mystery is included in the story.
