I'm quite new to the Index fandom, I just recently read through the Light Novel, so bear with me if I get something inconsistent.
The cornerstone of this fanfiction hinges on both Accelerator, ITEM, and Hamazura dying at the start of the story. So Accelerator will die against Touma in their first fight. Mikoto killed ITEM when they went against her in the start of the Radio Noise Sisters arc.
Hamazura will get butchered later on, or just disappear without doing much. Maybe he can live and do something, who knows — I like Hamazura a lot, but I am not a kind writer.
All main characters are already OOC from the start and that's due to the OOC Touma's influence.
So if you don't like your favorite characters dying at the start or OOC Touma and Mikoto, then you've been warned.
Other warnings:
—M rating, so gore and sex.
—Uber! Dark Mikoto, somewhat Dark! Touma, Chaotic Neutral Misaki.
—Touma is a Hentai protagonist with a harem.
—Main pairing: Mikoto x Touma x Misaki — Three-way relationship with a lot of Yuri between Railgun and Mental Out.
—Both Touma and especially Mikoto will be very dark throughout Old Testament but will start somewhat redeeming themselves in New Testament and start reforming towards chaotic good instead of chaotic evil as they are right now.
—They will grow up basically.

The main protagonist will be Misaka Mikoto and she will be taking over Accelerator's role and arcs in the story along with her own arcs in the Railgun series. Yes that means she will need to work hard to become a stronger and responsible Big Sis to both Last order and Worst.
The secondary protagonist will be Kamijou Touma and he will both deal with Hamazura's arcs as they come. He's quite different from canon Touma from the start.
The third protagonist will be Shokuhou Misaki but she won't have as many arcs where she is the main focus. She will work as the most important side character to Mikoto's story.
It will mostly follow the Canon timeline, but there will be a lot of major changes and subtle changes to the arcs.

She stood on the bridge.
Some memories were going through her mind. Of when this whole situation started.
Yes, this was started by her.
Misaka Mikoto, dumb naïve Misaka Mikoto, only 7 years of age, only but arrived at Academy City and dumb naïve idiotic Mikoto had given them everything they needed. Her DNA Map had been stored in the City's database. To get rid of that she would need to raze the whole City to ashes, and even then, her DNA Map just might escape to another lab on another country. It was an inescapable and unending battle. Her DNA Map was now there for every scientist and researcher with just a bit of clearance to peruse and abuse it.
The city protected them of any crimes against humanity and that's why her sisters were born. Originally to see if it was possible to mass-produce Level 5s. An understandable but grotesque way of thinking, at least the Radio Noise Project she could understand, build an army of Railguns and nothing in the world could oppose that.
However, that did not happen, her sisters were considered defective by the researchers for not achieving more than a Level 3. Mikoto grit her teeth at that.
Instead the project was scrapped, however unbeknownst to her, her sisters whom she had no idea existed until that moment had been used unto a far sinister project.
The Level 6 shift.
By massacring her sisters 20,000 times. Accelerator, the #1 Level 5 would theoretically advance towards as of yet unknown Level 6.
Mikoto hugged herself so hard the air itself around her cracked.
T—They want to turn that monster into a Level 6? For what? In the end, that murdering psychopath only cares about himself.
What would happen if he turned himself into the fabled Level 6? Would cancer be cured? Would world hunger end? Would people's lives turn out for the better?
Of course not, a psychotic mind, even with the power of God at its fingertips would only scoff at the suffering of others. Mikoto learned this well with her own hands in these two weeks of suffering. Even if he achieved Level 6 would the researchers really think he would let them study him? Accelerator? Him?!
If so, they are hopeless fools blinded by their greed and pride. In the end, nothing will be achieved once he reaches it. Nothing will be gained, only lost. Nobody will be saved; only more people would get hurt.
Mikoto became a devil after encountering Accelerator. After seeing her sister die in front of her, after seeing her sister's leg which was identical to her own. That was her own flesh and blood, her little sister. That was her leg…
Something finally snapped.
Ever since the start of summer back in June, ever since the meeting with a certain boy, something fundamentally changed within her. That certain boy was violent, always facing his problems with his fists, a total delinquent, but still retaining normal friends, a normal life and a bunch of girls falling over for him. He was the first person to pint out how she'd been stagnating with her power. That she became content in staying where she was. That once it became too difficult to progress her power in meaningful ways she had desisted, just because she'd been labelled a Level 5. That yakuza wannabe called her a stale bread. She wanted to laugh, laugh because it was true. She was the strongest Electromaster, nobody was above her in her field. So she stopped. All she needed were just a bit of convenient technology manipulation, electrical shocks and her Railgun. Before the start of the summer, she thought it to be enough.
However that certain boy opened her eyes and since then she grew exponentially stronger, as if something or someone stabilized her mindset again. Yes, it was thanks to that idiot and violent wannabe punk. He did not give her any suggestions or advice on her calculations because he was at the end of the day a Level 0 who barely understood Esper powers, what he offered her was a change of perspective.
He told her that she was the strongest Electromaster, so why the hell wasn't she living up to the title? That idiot offhandedly insulted her, saying that he could name five to ten comic heroes that controlled lightning who were stronger than her.
Even reminding herself of those days where she would spend the long days of summer with him, that idiot by the name of Kamijou Touma staved off her depression at this moment.
People often emulate those they admire. And she emulated Touma's level of violence for herself.
Even major problems that had arisen after that, such as the Level Upper and Poltergeist incidents had been easy. Mikoto became exponentially stronger than she was before meeting him. Even when Dr. Kiyama used multi-skill against her. Everything had been useless, entering a physical confrontation with Mikoto was one of the worst possible choices one could make once Mikoto learned how to implement her lightning to her punches and kicks. Even when that artificial fetus angel thing had been born. Everything had been useless; her Railgun became a weapon of mass destruction that could melt at the temperature of the core of the sun and her iron sand manipulation could cut an aircraft carrier in half.
Everything was easy.
Even when Therestina Kihara Lifeline had used advanced Mechs and Capacity down against her. Everything had been useless. She was past the point where interference could even affect her.
Ever since her perspective change, she had been looking into other fields of study to apply her powers as an Electromaster.
The first field was technology manipulation, she had refined this power to extremes she hadn't been able to reach before. Fruits of her power over technology had shown when she started investigating this messed up project. And Mikoto knew that this power could grow even stronger and lead her to an untold stage.
The second field she delved into was something that she hadn't even realized but now made much more sense. That was Bioelectricity. She had needed to study this by necessity, since the victims of Level Upper hadn't woken after the fact and the vaccine program from Dr. Kiyama had not saved the Child Errors. Mikoto used bioelectricity to mess with the signals sent from the body, everything was controlled by them and so she entered the minds of those victims and saved them.
Bioelectricity was something that caused her great sacrifices so she could learn it in such a short amount of time. The many times where she committed self-inflicted pain, lost one or two senses by accident, or almost died were immense, but she had to do it so she could save her friend Ruiko and save those Child Errors from Dr. Kiyama's memories.
Yes, her powers had grown immensely, over orders of magnitude even, yet they were nothing when compared to the monster known as Accelerator. There was an uncrossable chasm between the two of them in terms of power. No matter how much she improved, an Electromaster could only manipulate 95% of the universe. Accelerator could manipulate 100% out of 100%.
There was a fundamental difference just because of that.
Misaka Mikoto could not beat Accelerator, no matter how much she had improved.
That was the fact.
After some weak tutorial bosses, the first boss had been an impossible task in this harsh game called life.
If she could not beat him no matter what she tried, then the only choice was to shut down the project and save her sisters another way.
And thus. Misaka Mikoto became a devil.
She dragged her body on a pit of mud and blood, just for her sisters.
22 laboratories burned. 357 researchers had mysteriously been ripped and torn to shreds, killed and murdered, their lives broken, tortured, castrated, dismembered, decapitated.
In these past two weeks. Mikoto became worse than a monster at a slasher film.
What was the difference between her and Accelerator?
She really did not care anymore; he was a monster killing her sisters. And those scientist watching created that exact scenario, they enabled her sisters to be killed. She did not care about good or evil anymore. Her sisters were being killed and that was the only thing that mattered.
Hypocrisy?
Greater good?
Justice
Redemption?
Her mind did not care about any of it. Her sisters were dying and if she had to become a monster then so be it. Her life was worthless from the very beginning, decided by Academy City itself.
If it meant saving her sisters, becoming a psychotic mass murderer worse than Accelerator was easy as pie. It wasn't even that hard of a decision.
Mikoto became a devil just like that.
Even so, she believed that a life of a Level 5 was worth more than 400 researchers. Even if she murdered them in broad daylight, the board of directors would find a way to pull everything under the rug. That's how it worked in this city.
However, Mikoto would possibly be going to prison soon. After all Mikoto killed the #4 Level 5 Esper two days ago along with the entirety of their group. She killed the Tea-colored Meldowner called Mugino. She killed that creepy black-haired girl in a pink tracksuit called Takitsubou. She killed that hoodie girl with a nitrogen shield called Kinuhata. She killed that bomber bitch with the blond hair called Frenda.
The #4 really thought she was weak. The #4 really thought her laser-like attacks had no equal and that she could just brute force Mikoto into submission. The #4's power was of a similar basis to Mikoto's own.
And that what decided that fight.
I am the strongest Electromaster and that bitch thought I was weak. Mikoto knew that there was a significant power difference between the rankings of the Level 5s, she tasted that difference directly when she faced Accelerator.
Mugino who could only summon 20 or so baby-like 'lasers' hadn't expected Mikoto to just reverse calculate her entire power and summon 1000 larger 'lasers' after copying her power in the span of 20 or so seconds from the beginning of the fight. That was the end of the Meltdowner, killed by her own power stolen by somebody else.
Her power was to forcibly control electrons between the ambiguous state between particle and wave. I see, shooting it like a cannon would melt most things. That's why she became Level 5 even though her power is so restricted and narrow. If she fought me before I met that idiot, I would probably be dead instead.
But Mikoto was the strongest Electromaster, and now she worked hard to live up to that title. Any fight against her possessing the same fundamental powers would only end in defeat.
She sighed as a ball of energy was formed in her hands. Mikoto's was a dark blue, almost black instead of the green rays the Meltdowner produced. Mikoto possibly had ten to twenty times more fire power than Meltdowner even had with her own ability. And her control over it was likely a thousand times more.
There was no way to keep the death of the #4 under wraps, not when Mikoto had previously killed 400 scientists. The board of directors wouldn't be able or willing to pull this under the rug. Not when the level 6 shift project has caused such loss for the city, 400 brilliant researcher minds and a Level 5 lost. Tomorrow morning she would probably be hunted by Anti-Skill and Judgment for her crimes, her face would be plastered across the news or something.
Her friends would turn on her and the first person to hunt her down would be Kuroko.
"How did this happen?"
It was because of her. Because of her stupidity.
"Where did everything go so wrong?"
It was because this city was wrong. It was because the city decided that the only value her precious sisters had were their deaths.
"I just wanted them to smile, I just wanted them to be free…"
Mikoto's forehead circle from a mixture of deep sadness and depression. Nobody would help a person who would commit mass murder to save her sisters from another mass murderer. It was laughable.
She wanted to laugh until her throat bled.
Her fingernails scratched the steel bridge, breaking them as blood ran from her fingers. Her fingernails were immediately healed due to a 'consequence' of meddling with Bioelectricity, however the blood remained on her hand, so a bystander might think that she still was hurt.
Pure anguish filled her.
"H…Help… Please help me…."
Nobody would save a monster.
A mewl drew her attention.
A black kitten, passing through her legs. Black cats were usually a sign of misfortune, weren't they?
She was tired. Her brown eyes, full of life before all this happened, were now completely dull. This is it, the final night of her life. There was no way to run, even if she survived this night she would only be demonized. So she would face Accelerator one last time and end it all.
"What the hell are you doing?!"
