A/N: This is an idea I had and after the response I got from the question I asked on Reddit related to this, I thought I would wrote a fanfic about it. I do not know how long this will be. Might just be a couple of chapters. Depends on the response I get. The timeline in after Season 16, but I'm not mentioning COVID, at least in this chapter. Meredith and DeLuca have broken up and she and Hayes are not together yet.
Amelia was by the nurses' station in the E.R., when a finely dressed man approached her.
"Hi. I'm looking for Dr. Margaret Pierce. Can you tell me where her office is?" He asked in a European accent.
"Hi. I'm Amelia. Who are you, and why do you wanna talk to Maggie?" She asked curious.
"I'm Dr. Jonathan O'Neill, and I'm pretty sure that she is my twin." He said, Amelia almost dropped the tablet she was working on hearing this.
"You cannot be her twin." She said.
"I am. I saw the adoption directory." He replied.
"Oh God. You're non-identical twins." Amelia said. "Wait... That means..." It suddenly struck her that if he was who he said he was, then he was not only Maggie's twin, but Meredith half-brother and Richard's son. "Come with me." She paged Meredith and Maggie to an exam room in the E.R., where she also took the doctor standing beside her. "Go, have a seat, I will have to do a blood test."
"Why are you so.. I dont know, active, interested..?" He asked confused.
"Maggie is my sister" Seeing his eye enlarge she corrected herself, "Not biological, just as a belief." Jonathan let out a breath he didn't know he was holding.
When Meredith and Maggie came into the room, she stepped outside with the ladies adn explained them the situation. They both couldn't;t believe it and were not ready to take him on his word.
"Wait, I do not think he knows anything about you, Mer." She said.
"Okay..." Meredith nodded and they both entered the room to greet him.
"Hi. I'm Dr. Maggie Pierce." She smiled, still confused and on edge.
"Oh my god! Hi!." He excitedly got up and shook her hand.
"I'm Dr. Meredith Grey." Meredith greeted as well.
"I'm a huge fan Dr. Grey. I'm Dr. Jonathan O'Neill. I was truly amazed by your invention of mini livers." He said, shaking her hand too. Seeing and hearing him, they both were confused even more. Yes, he could be a non-identical twin of Maggie's, which explained his skin colour, but his accent wasn't American
Maggie sat on the other side of table, beside Meredith. Amelia took out the blood test kit and first collected Jonathan's blood and then Maggie's.
"Are you from Europe?" Meredith asked.
"Yes, my adoptive family moved to Switzerland when I was a kid. I got my Medical degree at University College London and did my residency in Neurosurgery in London as well." He explained. "I came across my adoption papers at my parent's house and so I contacted the firm in Boston, where I found out that I have a twin-sister. I'm hoping you can tell me a bit about our birth-parents."
'That explains a lot' thought all three ladies. Amelia asked for a nurse to take the samples to the lab and put a rush on them.
"Do you know their names?" Maggie asked.
"Nope. Just yours." He replied. Maggie tightly smiled.
Right then, Owen barged in. "Grey, Shepherd, Pierce good all three of you are here. We have a patient coming in, he was T-boned by an eighteen wheeler." Hearing this, Meredith and Amelia paused. "Oh shit. I'll page..."
"No. It's okay. Let's save him." Meredith said, determined. They nodded and followed Owen to the Ambulance bay. An idea struck Amelia and she turned to Jonathan, "You are a neurosurgeon, right?"
"Yeah."
"Follow me." She wanted to see if he was good. Maggie and Meredith's brother was to be a medical superstar as well.
They were just rolling the patient into Trauma Bay 1 and so they followed him. Amelia told him to take charge and so he started ordering tasks for the nurses and interns. Meredith worked on the abdomen and did an ultrasound while Maggie put in a chest tube to stabilise his breathing. Owen looked at the newest addition confused. Amelia pulled him aside, and explained. When she saw that he wasn't convinced, she promised that she would be there over-looking him the entire time, cauisng him to finally relent.
They wheeled the patient to the CT room for a full-body scan and the four doctors waited for the images to load.
"It's really an honour working with you, Dr. Grey." Jonathan said.
"Amelia... why?" Meredith asked.
"I'm going to be here the entire time. I want to see how good he is. You know, Maggie' twin." The two others understood what was going on.
"How are you their sister?" Jonathan asked.
"I am... was.. Meredith's sister-in-law." Amelia answered.
"You're married?" He turned to Meredith.
"My husband died in a car crash." Meredith answered. "He was T-boned by an eighteen wheeler."
"I'm sorry." He said. "Aah... that explains your reaction earlier." He pointed towards Amelia and Meredith. The two nodded.
"Okay." Maggie said. "Flail chest."
"Blood in the abdomen, third quadrant, I need to operate, now." She said.
"Brain bleed. God, it's getting worse by the minute." Jonathan said.
"Parker. Take him to the OR, now." Meredith ordered, getting up leaving the room. "I'll meet you in the scrub room." She said, before leaving.
She went to her office and dialled a familiar number. "I'm about to scrub in. What's up?" came the greeting.
"I have a brother." Meredith mumbled.
"You have a- what?"
"Half-brother, Maggie's twin." She replied. "And I'm operating on a guy that got T-boned by an eighteen wheeler."
"God, Mer. Ask someone else to scrub in. Bailey would do it."
"No. I need to... I need to save this guy." Meredith said.
"You sure?"
"Yeah... I'll call you after the surgery." She said.
"I'll stock my Tequila collection and video call."
"Done."
Meredith cut the call and opened the top drawer on her desk. It was the only space in her office which she kept locked. She took out the ferry-boat scrub cap ad gently touched it, remembering Derek. She put it on and then took out her regular scrub cap to put it on top of it. She did it during her diffiucult procedures, it gave her a sense of strength. She was going to need it today.
Maggie was freaking out as so, because Jonathan knew ony about her, and no one else. Boy was he going to be shocked when he would come to know about Meredith and Richard. They had decided to not tell Richard about it as he was still recovering from his surgery and did not want to cause him any unnecessary stress.
All four doctors were scrubbed in and prepped for the surgery. Amelia was there only as back-up.
"So, Dr. Grey, I read that you first started your residency in Neuro, I would love to pick your brain about it sometime." He said while operating.
"Yeah. I trained under my husband for four years." She replied.
"Wait... Shepherd right?" He briefly turned towards Amelia. "Your husband was Dr. Derek Shepherd?" He asked astonished.
"Yes."
"I was in awe of his expertise. I used his virus for malignant gliomas numerous times. Your name was in it too, right?"He confirmed.
"Yeah. We did it together my second year of residency." She replied.
"It would be a dream come true for me, to be able to scrub in with him." He said. "But, I am performing surgery with his wife and sister, so that comes pretty close." He added. "Is this hospital named after you or your mother?" he asked.
"Neither. It was named after my half-sister, Dr. Lexie Grey and friend Dr. Mark Sloan, both of whom passed away after being in a plane crash." She replied, taking a deep breath. She did not like to think about those days. Derek's accident, the plane crash... they brought up horrible memories for her.
"Dr. Mark Sloan, the plastic surgeon? My mother's sister consulted him once, amazing doctor." He said.
"The same." Amelia replied.
"Dr. Pierce, where did you go to med school?" He asked, wanting to know more about his twin sister.
"I went to Harvard." A nurse came into the OR and passed a sheet of paper to Amelia. She put on her head set with lights to read what was written and briefly smiled.
"Nice. So, Dr. Grey, have you started dating sicne your husband passed?" he asked. "I mean, I know I shouldn't ask especially when we're operating on this patient, but I would love-"
"You are going to regret asking that." Amelia interrupted. "He is telling the truth." She told Meredith and Maggie.
"Thank you for verifying that. But, why would I regret asking Dr. Grey out?" He asked. All the three of them looked at each other and Amelia took over from Jonathan, before Meredith replied.
"Because I'm your half-sister. Your mother was Ellis Grey." She said.
"What?!" He couldn't believe what he was hearing. "Oh God... I just.."
"Yeah," Meredith chuckled.
"I have two sisters?" He still couldn't believe it.
"Yup." Maggie replied.
"Wow! What about our dad?" He asked.
"That would be Dr. Richard Webber. He works here too." Maggie replied. Throughout the surgery, Jonathan kept asking them questions and they kept replying.
The surgery was a success and Meredith and Amelia were glad about it, a little more than the other two. When the four left the scrub room, they were met with Bailey and Webber.
"Who are you? And who gave you privileges to work in my hospital?" Asked Bailey.
"I did. Bailey, Jackson, meet Dr. Jonathan O'Neill." She introduced them. "Jonathan, meet our Chief of Surgery, Dr. Miranda Bailey and Head of Plastics, Dr. Jackson Avery." He shook hands with both doctors.
"At least he is a doctor. I still don't get why he was operating." Bailey said.
"He is Maggie's twin brother, and Meredith's half-brother." Amelia added. Both Bailey and Jackson were shocked at the revelation. "He is a neurosurgeon."
"Grey, you do have a lot of family." Bailey said, shaking her head. "Well, Dr. O'Neill, what are your plans?" Bailey asked.
"I... I don't know. I just came here to meet my twin-sister. I now have another sister as well as my biological father is here. I need time to digest that." He said. "Any other family that I have?" He turned towards his 'sisters'.
"Well, I do have three children." Meredith said.
"Derek Shepherd was my brother-in-law!" Jonathan half-shouted.
"Calm down, fan-boy." Amelia sarcastically remarked. Seeing Bailey and Jackson confused once again, she said, "He loved my brother's work."
"Who didn't? Dr. Derek Shepherd and in recent years, Dr. Meredith Grey. I followed them both closely." Jonathan replied.
"Hmm... well, Dr. O'Neill, once you have decided what you want to do, come see me in my office." Bailey said, walking away.
"Did you do the blood test?" Jackson asked.
"Yup." Amelia replied.
Suddenly Meredith's phone started ringing, she saw who it was, and then answered. "Hey"
"You have a brother?" the person asked
"Yes. He is a neurosurgeon." Meredith replied.
"You okay?"
"Yeah, I guess."
"I can be there in four hours."
"I'm fine."
"Yeah, you're fine. That's the problem. I need to be there. See you tomorrow morning." Saying that the phone was cut.
Meredith closed her eyes for a moment, thankful for her friends that were a call away from her and then turned to Maggie and Jonathan.
"You wanna come over for dinner?" Meredith asked Jonathan. She remembered this what Derek had done when Maggie had come to Grey-Sloan, so she thought it was a good idea.
"Yeah, sure." Jonathan happily replied.
"Amelia, you and Link come over as well?" Meredith offered.
"Cool." She replied. "We can order Italian." Maggie nodded in agreement, and they told Jonathan to meet them in the lobby while they went to their offices to close up for the day.
