The basic explanation is that Nico died, and Maki's idol girlfriend named Yazawa is actually Cocoro, the 2nd child in the Yazawa family. But that's not the end of the story.
All of the above: Coffeeshop Date in its new slot on fanfiction (or AO3) is where you will find the story you reviewed and its (so far) two continuations.
2. One Day in Akihabara is also its own story. The story "Come to Bed" is the continuation, and this story is Completed.
