Description of incident: A breach of containment occurred this week. All sensors and cameras inside Subject #u0n's containment cell mysteriously went offline. During the ten seconds that all monitoring devices were disabled, she broke out of containment. No visible damage was observed on the outside of the containment which til now stumps the mind. Subject #u0n then proceeded to disrupt as many of the security measures within the facility as possible using multiple EMPs. Fortunately, only one of the subjects broke out of containment. Unfortunately, it was Subject #0034. Even at such an early stage of development, #0034 is extremely powerful and surprisingly persistent. During the breakout, #u0n and #0034 were seen conversing on a security camera that had managed to still function after the EMP. Both subjects went directly to this specific spot, almost as if it was a rendevous point. This implied that this breach was planned by both of them. Aside from the implication that subjects have been conversing under my proverbial nose when they are not allowed, it means that #u0n somehow managed to communicate with #0034 despite all attempts at communication with the subject yielding poor results. Such marvelous intelligence wasted on troublemaking and illusions of freedom. #0034 was eventually detained and placed into a temporary cell. #u0n managed to escape capture but all exits were put on lockdown, meaning that #u0n is still in the facility.

Follow Up Action
Description of actions to be taken: More intense discipline regime might be needed to extract compliance from current subjects. The facility must be put on maximum security to ensure #u0n doesn't leave or cause any more breaches. #0034's containment cell has now been deemed unsatisfactory and will be transferred to a more secure location.
