When the time came for the surgery, Regina was clad in scrubs with a scrub cap, covers for her shoes and a face mask. Emma was given her epidural a while previous and was pleasantly not uncomfortable. In the OR, Regina was given a chair next to Emma's head and she held the other woman's pale hand in her own while using her free hand to stroke blonde tresses. There was a piece of fabric hanging from a clothesline that separated her chest and up from the rest of her body, preventing them from seeing the operation.

"Are you ready?" Dr. Whale asked Emma. She nodded and tried to offer a weak smile. Instead it came across as more of a grimace and Regina placed a light kiss on her girlfriend's lips.

"Hey, you're going to be fine, sweetheart," Regina whispered. "I love you."

"You too."

"Dr. Whale?" Regina spoke.

"Yes?"

"After he's born, can Emma hold him? On her chest I mean. I've read that it's a really good way to bond."

"Yes, certainly. Given that the baby is fine and nothing is abnormal, that should be completely acceptable."

It only took half an hour before their baby boy was out. Regina was allowed to cut the cord and after they cleaned him off, weighed him and measured him (he was 18 inches long and 5 pounds, 1 ounce) Regina was handed the baby to take over to Emma. Each time he opened his eyes it was only for a fraction of a second, but they were big and the dark blue that most babies are born with. His hair lay in thick and curly tufts in a light brown color. His nose was tiny, but Regina didn't recognize it as her own nose, but she saw her cheekbones in his face. She brought him over to the blonde, lowering her gown down and laying him in the center of her chest. Emma didn't look at him, actually she looked away, which broke Regina's heart.

And then the most amazing thing happened. The baby, on instinct scooted himself along Emma's chest until he found her breast and he attached to her nipple. The sensation shocked Emma, causing her to gasp and look down at the baby suckling her. Her body responded the way it should have and the colostrum came out, but not much since it was still early for the baby to have arrived. After the baby stopped suckling, Regina took it as a sign that he had gotten everything out of that breast and lifted him up, resting him against her shoulder and gently patted his back to coerce out a burp. After he gave a tiny one, she noticed that Emma had been watching her with wide eyes and small tears.

"Do you…do you want me to put him by your other breast to feed more?" Regina asked hesitantly, not wanting to upset the blonde. To her amazement and pleasure, Emma offered a weak smile and nodded, so Regina lowered him against her other breast and watched him attach on his own, once again feeding from her while the doctors were finishing the surgery. He didn't eat long on that side either, so again Regina picked him up and burped him before laying him back in the center of Emma's chest, resting her own hand on his tiny back.

"We haven't talked about names," Emma whispered.

"No, we haven't. Do you have any ideas?"

"Bentley?"

"Yeah? Bentley…Daniel?" Regina asked. Emma let out a giggle and smiled.

"No." Regina's face fell.

"Um...what middle name would you choose?"

"It's not the middle name I was laughing at, babe. I'm sorry. I'm just thinking, his last name is going to be hyphenated, right?" Regina nodded, uncertain of where the blonde was headed with her question. "So you want his name to be Bentley Daniel Swan-Mills?" Regina shrugged.

"I don't know, I guess. What's wrong with that?"

"So his initials would be BDSM?"

Regina's eyes widened. "Oh, you're right, no. No, that won't do."

"So…we will use neither name, and we'll both pick something else?" Regina nodded in agreement.

"You should pick his first name, though. I named Henry, after all."

"What about Grayson?"

Regina smiled, "I like that, sweetheart. Grayson it is."

"So?" Emma prompted, running her fingers along the baby's head. "What middle name?"

"What about Emmett?"

"Emmett?"

"It's the closest thing to male version of Emma."

"Regina, you don't have to do that."

"Sweetheart," Regina said, dropping a kiss to Emma's forehead. "You have been such a fighter this entire pregnancy and I think that it would be a beautiful name, Grayson Emmett Swan-Mills."

"I like it."

"Me too. Are you doing all right? I think your family is probably dying for an update."

"Yeah, I'm good. You should bring him out to meet them."

"Are you sure you don't want me to leave him here?"

"Yeah, I'm sure Henry is excited to meet his brother and my parents and everyone probably want to see him."

"Ok." Regina asked the nurse for a blanket, so the nurse took the baby and swaddled him, giving him a little blue cap to go on his tiny head. When Regina had the baby cradled back in her arms she walked out to the waiting room where everyone was chattering and it took a moment for anyone to notice her. Henry was the first to jump up excitedly and Regina tilted her shoulders to show Henry his little brother's face. Everyone else had gathered around and Regina tilted him each way to allow everyone to see him.

"He's beautiful, Regina," Snow cooed.

"He is. I think he's a pretty good mix of Emma and me."

"He is. He has the exact same nose as her. Your cheeks, though." Regina smiled lightly.

"Everyone meet Grayson Emmett Swan-Mills. I'm going to take him back to Emma, but once she's in her room and sleeping we'll get visitors in there to hold him."

The rest of the surgery only took another hour and Emma was back in her room sound asleep. Henry, David and Snow were in the room with her, taking turns holding the baby while they hung around to keep an eye on Emma. Regina left to find Dr. Whale.

"Regina," he called from behind her. She turned around and saw the blonde doctor she had been looking for.

"I was just searching for you."

"What can I do for you? You look…frantic."

"When I laid Grayson on Emma's chest he wiggled around until he found her breast and she breastfed him. And something shifted in her. She didn't even want to look at him, and when he started eating, something…something in her eyes changed and she wanted to do it. And I think she is going to want to breastfeed."

"And you're worried because then we shouldn't put her on medicine."

"Yes. Thank you for understanding. What other options do we have? I think…I think breastfeeding will be good because I saw how she reacted to it, and hopefully it will help her bond with him and help her feel more like herself. But…we need something, another route to get for helping her feel better."

"Does she drink tea?"

"Yes. Why does that matter?"

"There an herbal tea, it's called passion flower. It also comes in pill and oil form. It's supposed to be a natural cure for depression. It helps some people, and doesn't help others. But…it's worth a try."

"Thank you. Is there anything else?"

"Not off the top of my head, but I can research with the mental health department and a pediatrician."

"Thank you, Victor, for taking care of my family."

"Of course, Regina." He nodded and headed off down the hall, leaving Regina wondering when he had changed his tune.

By time nine rolled around that evening, everyone had left, letting Regina, Emma and Grayson have some bonding time before the baby fell asleep. Emma had breastfed several times throughout the evening, supplementing only a little bit with formula, and it seemed to really help her. The two women were curled up in Emma's bed together, Grayson resting between them, his back on Regina's thigh and his butt propped up against Emma's hand. Each woman was holding one of his hands and they had their heads pressed together.

"We made him," Emma whispered.

"We did. We made him out of True Love, which means he is strong like you. I hope he is so much like you, Emma."

"Why?"

"Because you're a fighter. A survivor. He is too; he's already proven that just by being healthy enough to live this early on and to be able to be here in our arms instead of in an incubator. He is an amazing baby."

"Regina?"

"Yes dear?"

"I want to breastfeed him."

"Right now?"

"No. I mean I don't want to take medicine that will prevent me from being able to feed him. When I'm feeding him…it gives me some time with him, to bond and I want that. I don't want to be one of those depressed mothers who can't look at their child. I've seen it and I don't want to be that, it's too sad. Feeding him helps."

"I know. I could tell. Dr. Whale mentioned a natural herbal tea and oil and vitamins that you can take and we can see if that helps."

"Really?" Emma lit up like she hadn't expected Regina to agree.

"Sweetheart, I would never take your choices away from you. Not ever. Ok?"

"Ok."

When they woke up the next morning, neither woman was refreshed. Grayson hadn't cried too much, just when he needed to eat and Emma had been able to feed him two of the three times, but the time between those two he had a small bottle that Regina had made and fed him. They were both exhausted but neither woman wanted to send the baby to the nursery, they didn't want to take their eyes off of him. Snow and David came, bringing breakfast and Henry. Henry was so thrilled about his little brother that he kept bugging to hold him. Regina, being the worrier that she was, made him sit in a chair and she brought the baby over, setting him properly into Henry's arms.

"Now, remember what I told you about his head?"

"Yes, I can't let it fall backwards."

"Very good."

The adults chatted for a few minutes before Henry spoke up again.

"Mom?" he asked. Regina looked up at him and smiled.

"Yes sweetheart?"

"Does he look like me?"

She smiled at his innocent question. "Well, you have your father's nose, but he has Emma's. You have Emma's eyes, but…we don't know what color his will be yet, most babies don't have their proper eye color for several months, but his eyes share the same shape as yours and ma's. I think he'll probably look like both of you with that feature. But I think he has my chin, and my cheeks. But he definitely has the same skin tone as you and Emma. So…he looks a little bit like you, but I think he looks a lot like me."

"Oh, well that's cool," he said with a shrug. "Um, can I ask another question?"

"Yes dear, of course."

"You guys hyphenated his last name, right?"

"We did."

"Uh…um, can I be a Swan-Mills, too?"

"Of course you can. We'll get the paperwork taken care of as soon as we get settled back home."

"Hey…what about the nursery?" Emma asked.

Regina walked over to her, carrying Grayson back so he could see his blonde mother. "Actually, your mom, dad, Ruby, Tink, Neal, Victor, Belle and Rumple took care of it."

"They what?"

"Well it seems while we were here, and in between visits with them, your—our family and friends were shifting and moving our house to make room for the baby."

"But we didn't buy anything."

"David and Snow, and Rumple bought most of it. The other girls contributed with some very darling clothes I will have to show you when we get there."

"You guys didn't have to—" Emma started to say to her parents.

"We didn't have to, no. But we wanted to," Snow stated. "There wasn't much we could do to help you guys out since you've been in the hospital and parents want to help, so we helped."

"Thanks mom, dad. That was really nice."

"So how did you guys choose Grayson's name? I really like it." Snow offered a small smile to the other two women.

"I picked Grayson and Regina picked Emmett."

"After Emma," Regina added.

"Yeah, and I don't know, I remember hearing the name Grayson somewhere, I just don't remember where."

"Well, it's lovely."

They visited a while, and the family of four had many other visitors that day, including Neal. When Emma's ex came in, Regina offered to let them visit for a few without her, but Emma really didn't want her to go. Instead, Regina sat with Henry on the far side of the room while Neal visited Emma and congratulated her on the baby.

"You know, Em, I know I wasn't really supportive of the idea of you two…because I still loved you and all, but I want you to know that I think she's good for you. And you're good for her. You guys really seem happy, and Henry is happy about it. I can't really be against something that makes two of the most important people to me happy."

"Good. And…you'll find something."

"Yeah. I started dating someone. It's still early, but she's pretty amazing."

"Neal! That's great!" Emma encouraged. "I'm really happy for you. Are you going to tell me who she is?"

"Um…Tink?"

"Shut up! Good, now I can keep her away from my girl."

Regina chuckled against the other wall, having heard their conversation. "Tink doesn't hold a candle to you dear, so you wouldn't have to worry about her anyway," Regina teased.

"Alright, well, I'm going to get out of the way here. You guys want me to take Henry?"

"Henry? Would you like to go with your dad for the night? I can call your grandpa and tell him he doesn't have to come get you tonight."

"Yeah!" Henry said enthusiastically. He still didn't have his memories back from before the Sidney incident, but he was forgiving anyway when Emma and Regina told him the situation of what happened. Apparently resenting Regina for a long time was too much resentment for a child to handle and it didn't allow room to resent anyone else.

When everything was settled for them to spend the night with Grayson by themselves, Emma and Regina cuddled into the bed together and held Grayson between the two of them, much like they had the night before.

"Are you feeling alright, dear?" Regina asked in a voice just above a whisper.

"I…I feel better than I did a few weeks ago, but I don't feel completely like myself, no." Emma frowned. She made a face that looked regretful.

"You don't have to be completely back to yourself yet, sweetheart. It's completely normal and it happens to a lot of women. But the change I've seen just in the last week is a good sign." Regina leaned her head in to kiss Emma's temple. "I mean…look at him. Look how perfect our little creation is."

"Yeah, he was worth the pain…I mean, it sucks being so sad a lot, but Grayson is so perfect."

"Indeed he is."
