Episode 12 is where Dr. Shamal ended up lip to lip with Tsuna after being tossed by Bianchi (Shamal had tried to kiss Bianchi before being tossed at Tsuna).

"Tsuna, Who gave you your first kiss?" Yamamoto asked calmly and didn't seem to notice Gokudera's annoyed expression.
"You have no right to ask the tenth that kind of question." Gokudera said angrily, but then noticed Tsuna looked a bit twitchy and he walked up to him. The expression of annoyance towards Yamamoto was replaced with concern for the tenth.
"What is wrong?" Gokudera asked worriedly, Yamamoto was still smiling, and Tsuna almost cried at remembering on how his first kiss had been stolen. "Tenth? Tenth?"
"Just remembering it was terrible, horrific, and I had cried." Tsuna managed to say out loud and he looked at the ground. "It was Shamal."
Gokudera was shocked for a moment before his face turned into a perfect picture of rage, teeth showing, and his fists clenched. Yamamoto blinked for a moment and he looked at Tsuna.
"I will kill that fucking old pervert for kissing the tenth and ruining his first kiss!" Gokudera yelled angrily, his eyes were blazing in anger, and he wondered where he had been. "Where was I at tenth? I would have protected you."
"On the floor and ill for your sister had shown up." Tsuna commented to him and sighed softly. "I was a bit injured from your bombs and laying on the bed. When your sister tossed him and he ended up lip to lip with me. Stealing my first kiss by mistake."
Gokudera dropped to his knees and looked at the ground with depressed eyes.
"I'm sorry for failing to protect you, Tenth." Gokudera said loudly and he couldn't look at him.
"It is not your fault." Tsuna stated firmly to him and Yamamoto looked at them.
"If I had not thrown that bomb at those bee's that were chasing you." Gokudera said in a depressed voice while wondering if things could have happened differently. "Then you would not have been injured and vulnerable. I'm a failure."
"I do not blame you." Tsuna told him bluntly and he couldn't understand why Gokudera was beating himself over this now. "You are not a failure."
"If only I didn't have that stomach ache from hell due to seeing my sister." Gokudera commented angrily to himself as he hit his fists against the ground while silently cursing himself for that one weakness. Tsuna grabbed his hand, managed to pull him up, and looked him in the eyes.
"It is best to forget it and move on." Tsuna said to him calmly while silently hoping that Gokudera will drop the subject. "I'm planning to forget it again."
"Don't worry I won't fail to protect your lips next time. You can count on me for I'm your right hand man." Gokudera stated firmly, his expression was serious, and Tsuna felt a bit frightened not comforted at all. "Anyone that goes near them will die by my bombs. Don't worry because I'll protect them with my life."
'That old pervert is lucky that he is in Italy right now.' Gokudera thought to himself and he clenched his fists. 'However when or if he comes here to Japan. I will kick his damn ass for kissing the tenth on the lips.'
