Perkins can't clear her for surgery because she's not being honest. Here's the thing, though: there's honesty with other people and there's honesty with yourself.
Right now, if she told Derek about the baby; if she relived in words, out loud the heart pounding, blood ringing helplessness of watching him get shot, waiting for him to live or die? Now that would make her unfit for surgery; that might leave her on the floor of the OR weeping or screaming or whatever it is he's afraid of.
Not being honest? She's used to it, she's comfortable with it. It's survival.
