The hospital had to hold Twilight there for a couple of days. After multiple tests the doctors determined that she was healthy, but her pregnancy was "irregular". Twilight was told that she would have to stay in the hospital for the first few months of her pregnancy.
Luna was worried, but Twilight's main doctor assured her that there was nothing to worry about; they would get her pregnancy back on track in no time.
For the first month, Twilight was restricted to her bed. She wasn't allowed to leave her room and the only time she was allowed to get out of bed was when she needed to relieve herself.
Luna was as busy as a princess could be, wrapping up some major affairs that her and Celestia had been working through for months now. She visited Twilight and stayed for as long as she could during each visit before her work would take her away.
The doctor prescribed Twilight some meds that had to be taken daily. The medicine was designed to help the foal catch up to it's normal growth rate, but somehow even with the medicine, the foal was still a little behind.
Month two reared around and Twilight was still stuck in her bed. She started to develop a crazy appetite for hay mixed with chocolate, fish, and apple slices. She asked the doctor if she could have some, but he advised against it, claiming that it would not help the foal in its current condition.
He restricted Twilight to a strict diet of oats and honey. Twilight enjoyed the pair, until it became repetitive and she had to eat it every single day for breakfast, lunch, and dinner. She was also restricted to drinking water or milk.
Twilight was on the verge of losing her mind. Her body demanded cravings she wasn't allowed to have. Luna's visits became a little less frequent as her and Celestia finished their royal affairs.
In their absence Discord visited Twilight. He would make her laugh with his chaos magic and pull pranks on the doctor. He would even give Twilight chocolate milk.
"Hey, he never said the milk couldn't be chocolate," Discord chuckled.
Twilight was thankful that he was able to help her get her mind off of her cravings.
The third month had approached and Twilight was now allowed to walk around. The doctor said that walking would do her some good, but she wasn't allowed to fly and only allowed to use her basic levitation spell.
Twilight would walk around the hospital garden about three times before she would grow tired. She would then retreat to her room where she would be eating her oats and honey before taking her medicine.
The doctor started to prescribe a higher dosage of the medicine. This medicine would usually make Twilight drowsy and sleepy. She started to sleep more and for longer periods of time.
Celestia and Discord would come and visit Twilight, Celestia explaining the reason for Luna's absence. She didn't go much into detail, the only thing she said was, "Luna's been busy at work. You may notice some changes have been made whenever the hospital releases you."
Twilight grew ever more excited and a little impatent as she really wanted to be home with her wife. Celestia made sure to bring Twilight plenty of books to ease her boredom. Some being storybooks, other were parenting books. The more distracted Twilight was, the more likely time would fly and she would be released.
Month four came and at this time Twilight felt the weight gain. She wasn't showing yet but when she stood she felt more weight pushing down on her hooves. Her diet had been expanded to hay, but the hay was plain without any flavoring allowed.
The doctor found out about the chocolate milk and he scolded Twilight for ruining her diet. She apologized and the doctor said that he would keep a closer eye on her and her visitors.
This was the first month her friends had visited her. She was so excited that she made sure to get out of her bed and hug them all. When Twilight asked how they had known about her pregnancy they admitted that Luna has sent them a letter.
Twilight hadn't noticed this when they walked in but Fluttershy was carrying a little bundle;a bundle she only recognized when it sneezed. She looked over and Fluttershy carried the bundle to Twilight, moving the blankets and revealing the little foal who was deep asleep.
Twilight smiled as Fluttershy let her hold the foal. It was a little colt with a sky blue coat and a light blue mane. She smiled as the little pegasus yawned and opened his little soft blue eyes.
"His name is Tundra," Fluttershy whispered gently.
Rainbow explained how she proposed to Fluttershy only a short amount of time after Twilight's wedding. Mare Mayor married them without hesitation, but regardless of that, Fluttershy and Rainbow still got looks from certain ponies about their choice.
Rarity spoke about how her and Applejack were now dating. Applejack had admitted her feeling for Rarity and she instantly fell for AJ.
Rarity boasted Applejack a little bit by telling Twilight how much of a gentlemare she really is. Twilight listened carefully but in the back of her mind she couldn't help but feel like Spike might be a little crushed.
The girls brought Twilight some parenting books from the Golden Oak Library. Twilight thanked them and would often tell them some of the stories she has read, while thedoctor kept a close eye on Twilight.
Month five arrived and the doctor told Twilight that she would be released soon. This made Twilight happy considering she missed her home and her wife. Luna stopped by and started to visit, there were even times where she would spend the whole night.
Twilight was happy that they were finally able to cuddle again. There would be times that Twilight would get bad cramps and Luna would sit there and gently rub her belly. Twilight's belly started to grow more. It was still relatively small for how long she has been pregnant, but she was still growing.
Month six, and Twilight was finally released from the hospital. The pregnancy had been announced to the public, so paparazzi were all over the hospital. Luna was able to shoo most of them off and get Twilight safely home.
Twilight entered the room and immediately saw what Luna has been working on. There was a foal crib, a highchair, a changing table, and a smarty pants doll. Twilight wiped the tear that started to roll down her cheek.
Luna smiled, "Do you like it? I wanted to make sure that our foal had the best."
"I love it." Twilight said as she hugged Luna.
"But you do realize that our foal won't be born for a few more months right?" She asked.
Luna nodded, "I am aware, I just wanted to be prepared."
Twilight smiled as she pulled Luna in for a gentle kiss.
Month seven flew by as Twilight slept for most of it. Anytime that she was awake she was eating. She was finally allowed to satisfy her weird cravings without the doctor watching over her, and Luna helped her.
Month eight came before they knew it and Twilight started to become very grumpy and emotional. She would yell at anything and everything. Luna would apologize even though she knew she didn't have to.
Twilight would also cry over nothing. She was reading a cooking book and she started to cry over the fact that the corner of the page was torn. Luna would facehoof and blame Twilight's crazy emotional rollercoaster on her pregnancy.
Month nine was here and her friends wanted to get together and start planning a foal shower. Twilight really didn't want them to go through all of that trouble but they offered it anyway and wouldn't allow her to deny anything of their planning.
Twilight wasn't allowed to know any of the foal shower plans because it was apparently a surprise for her.
At this time Twilight was now bigger and actually looked like a fully pregnant mare. She couldn't walk anymore as much as she waddled everywhere that she needed to go. Her hooves would hurt if she walked too much, but she needed to walk because it would help her get used to the extra weight.
Twilight and Luna went to the hospital to get an ultrasound done and they realized that their foal was then a colt. The doctor congratulated them as he prescribed Twilight some more medicine in a higher dosage that would help with their son's development.
Month ten, Twilight has all but memorized all of the parenting books. She started to grow nervous at the thought of giving birth. Most of the books she read described it as painful. There was medicine that she could get and would help numb the pain, but Twilight felt like she had taken enough medicine.
Luna could tell Twilight was nervous and she tried to put her at ease,placing her hoof on Twilight's.
"Don't worry my love. It might be painful, but it will all be worth it once you hold our son in your arms."
Twilight smiled as she brought Luna's hoof to her muzzle and gently kissed it, "You're right."
Her friends came to visit her at the castle a few days before the foal shower. By the look on Twilight's face they could tell that she was nervous, exhausted and sore.
Rarity came up with a brilliant idea. The girls were going to treat Twilight with a trip to the spa and Twilight could finally get a few hours of rest and relaxation.
Luna nodded. She agreed that this spa day would do Twilight some good, and she knew that spending time with her friends would make her happy. She also knew that her friends would keep her and their foal safe.
The offer was extended to Luna for her to join them. Luna shook her head andTwilight looked a little hurt.
"It's just you don't get to spend quality time with your friends like you did before. I want you to enjoy this day with them. I will be fine here andcatch up on some reading." Luna said.
Twilight was still a little hurt but she understood why Luna wanted her to be with her friends. She leaned over and gave her wife a gentle kiss. Her friends waited outside and once Twilight joined them they were off to the spa.

The walk to the spa was a slow one but it gave the girls time to talk.
"So are you excited? You are going to be the first mare ever to give birth to a foal born of two mares." Rarity said.
Twilight nodded, "I am excited, but not because of that reason. I just can't wait to be a mother."
"I am so excited to be an auntie...AGAIN!" Pinkie yelled.
"Just for the record Twi, I think I will be teaching your little one to fly. Nothing against you or Luna, but I am definitely the best flyer in all of Equestria." Rainbow said.
Fluttershy giggled, "Rainbow don't forget we have to check in on the babysitter's to see how they are handling Tundra."
"Babysitter's? Who did you get?" Twilight asked.
"Who do ya think?" Applejack said.
"The CMC's of course." Pinkie said.
"Well if they are up for the challenge, maybe we can have them travel here so they can babysit for me and Luna." Twilight said.
"You know they would. They will do anything they can to leave ponyville, even if it's only for a few hours." Rarity said.
Twilight whined in pain as she felt a huge cramp attack her. She had some cramps before but this one was more severe than the ones before it. The girls all froze.
"Are you alright, Twilight?" Fluttershy whispered.
Twilight nodded, "I just had this really huge cramp that is all. Maybe the foal is hungry."
"Have ya had these cramps before?" Applejack asked.
"Well, yes. Luna and I just dismissed it as the foal being hungry." Twilight said.
The girls observed Twilight. Her belly was round and it looked like she was ready to give birth right then and there.
They finally made it to the spa. Twilight took a herbal bath that would help relax her muscles and her hooves, whileRarity relaxed as she slowly slid into the mud bath. Pinkie sat back in the chair as one of the spa ponies started to file her hoof. Applejack and Fluttershy were on beds, laying on their stomachs as they got massages. Rainbow just relaxed on the side reading a Daring Do book.
Every so often Rainbow would look over at the mare messaging Fluttershy and give her a dirty look when she was occupied.
Twilight relaxed as she felt her entire body just take a long, deep, sigh.
"It feels good to have my muscles relax." Twilight said.
Applejack sighed, "You said it."
Rarity had the cucumbers covering her eyes. "I agree. I have had this ginormous order that took an entire week to fill."
"Well Fluttershy and I have had our hooves full. Tundra finally got his little wings going. I have had to collect him from the weather factory twice." Rainbow said.
Fluttershy giggled, "Then there was that time that we found him caught in the beehive. The bee's were really nice and forgiving. The queen bee said that she understood, she has millions of children a day."
"I can barely handle our son. There is no way that I am having a million foals." Rainbow said.
"Not even if I want more?" Fluttershy asked as she hid her face in her mane.
Rainbow blushed as she hid her face in her book, "I will think about it."
Everypony laughed. Twilight closed her eyes for just a moment until Rarity let out a bloodcurdling scream.
Twilight opened her eyes and realized all of the girls were staring at her.
"What's the…" but before Twilight could finish her sentence she looked down at the water.
It was a bright red. She felt her hoof around in it, utterly speechless. The girls all stared, frozen in place like statues.
"This is...blood."

Luna was flying full speed to the hospital while Twilight was being rushed on a stretcher. Luna was able to get next to the stretcher and be with her wife.
"Twilight! Twilight!" She screamed.
But Twilight's eyes were closed shut. She wasn't moving at all and her breathing was weak.
"Twilight!" Luna yelled once more as a nurse pony grabbed her and pulled her away as the rushed Twilight to surgery.
Luna cried as she watched her wife disappear behind those double doors.

Luna and the girls were in the waiting room, the former had her face in her hooves and she was sobbing.
The girls all had her in a group hug as they waited for the doctor to come out and give them news.
It took hours, but finally a doctor emerged, a clipboard held in his hoof.
"Princess Luna," He called out.
Luna stood and left the girls in their seats as she went up to talk with the doctor.
"Well, Twilight is fine. She has made it through the surgery just fine. We had to give her 4 pints of blood because she had lost so much. Those is the good news. Now for the bad ones"
As the doctor continued, Luna immediately lost her composure as she fell to the floor. She was crying in both pain and happiness.
The girls ran up the Luna and collected her as they carried her to her seat.
"Luna what happened?" Pinkie asked.
"Is Twilight okay?" Fluttershy asked.
"Is the foal okay?" Rarity asked.
At the mention of the foal Luna broke into tears again. The girls hugged and comforted Luna as they waited for her to calm down again.
The doctor walked over the group, "If you want, you may see Twilight. She has been moved to her room."

The doctor led them to the room. The scene before them was absolutely heart shattering.
Twilight was hooked up to many machines. There were IV's pumping blood into her body while her eyes were barely open.
"Luna?" She called out in a raspy voice as she reached out for her with a shaky hoof.
Luna was next to Twilight's side, holding her hoof in seconds before speaking softly, "I don't blame you. It's not your fault."
Twilight sniffled, "It is my fault."
Luna shook her head, but Twilight nodded.
"I lost the foal."
