Summary: Set in S8 in the future. After everyone has moved on, Bonnie and Elena are dead, and it has become peaceful without any danger in Mystic Falls, Damon's still going through the loss he feels. Going through emotional pain after everything, but also having nothing to do, he decides to find out where Katherine was taken to all those years ago, since he doesn't believe that she's truly gone. Once he finds out that she's going through her own torment and torture in a prison world, he saves her life by getting her out of the prison world. When she's well again, what will ensue between the two of them? Will their love story continued, as it should have a long time ago? Based on "Little Do You Know" by Alex & Sierra.

It was a dismal night, as Damon walked through town, a bottle of Bourbon in hand, as usual. Everything had gone through hell. They'd gotten rid of Katherine after she'd tried to seek revenge by killing off the entire town, but Stefan had killed her and himself to stop her. They'd both gone up in hellfire. Now, after that, everything was peaceful. Even the heretics were gone, but Elena had died of old age recently. All of this peacefulness and his emotional pain and suffering made him want Katherine back. At least she'd given him a distraction from his pain before. He was alone now though, and he didn't know what to do. He wanted the pain gone, or at least the pain to become numb, rather than eat him up. Besides, when Katherine wasn't destroying lives, she was quite fun to be around, and she knew how to have a good time. He wished he could bring her back, but he didn't know how to. They all thought that destroying hell would mean that there was no such thing as ghosts anymore. However, he knew that there was one, maybe two prison worlds left. If Katherine wasn't disintegrated, she was in a prison world.
After thinking through all of this and, needing her distraction, even though he knew it was probably a bad idea, he decided to bring her back. If she was in a prison world, he was going to bring her back. For once in a very long time, he was going to save her from whatever torment she was enduring.

Meanwhile, Katherine was enduring never ending torment and torture. It was like hell, but it wasn't really hell. It was a prison world that brought your fears to life, as well as tormented you with the memories of those you couldn't save, as well as those that you regretted doing harm to. She tried to fight through it all, just like she always did, but she didn't know how long she'd been here, nor how to fight through this. The prison world seemed to know her inside and out. It wasn't letting her win the fight like she wanted to.
After finding an ascendant after finding out what prison world Katherine was in, Damon found his way into the prison world. It was different from the last one he'd been in with Bonnie years ago. It wasn't Kai's prison world, but he did see that Kai was there. It was different, because it was dark and gloomy, and there was a real prison there. It looked like some kind of abandoned town, but it didn't look anything like Mystic Falls.

After quite a while of the torment and torture, which involved not only fears coming to life, but also being injured from visions of those dead that weren't actually there, she didn't have any fight left. At the moment, all she wanted was to be home, instead of here. She just wanted all of this fear and pain to stop, but knew it never would. However, she didn't know that she was about to be rescued, for once in a very long time.
With those thoughts, she closed her eyes, letting the darkness engulf her, not caring what would happen right now. She'd had a lot of pain and suffering in her lifetime, but never like this. It was paradise next to what she'd been enduring in this prison world.

When Damon finally found her inside the prison, he also saw a lot of supernatural beings, all inside prison-like cells. He didn't care about the others, though. He only cared about her. So, without caring, he forced the cell door open. He then knelt by her. That's when he saw the damage, and knew he'd gotten to her in time. She was quite injured, so he took her jacket off and used it to tie around the wound on her leg, which was the worst wound, since she'd lost a lot of blood from her leg artery. She was still alive, which he was thankful for.
"If you can hear me, you're going to be okay, Katherine. I'm right here," he said. Then he gently picked her up in his arms, got out of the prison world, and blurred her to the hospital. It had been several years, but Meredith had been able to get back into being a doctor again ten years after they'd lost Stefan and Katherine, so he was going to take her to her. He may have nicknamed her "crazy doc," but he trusted only her when it came to those that were supernatural. Only she would be trustworthy enough not to do anymore damage than there already was.

Several hours later, she was in the recovery room after surgery, Damon seated on her bedside, and had just woken up to see him there, and also to see where she was.
She sat up in bed.
"Thanks. You saved me. How'd you know?"
"You're welcome. I didn't know you were in danger, but I figured I'd get you out of wherever you were at. It's a little quiet without you here. Elena's gone, Stefan's gone, and even Caroline has moved on from this town. I have no one left, so I have no purpose apart from you. Besides, seeing you like that, well, let's just say you didn't deserve anything like that. Let's just say I missed you, and leave it at that. Shall we?" he replied.
She saw right through him, though he hadn't really admitted it, and knew that he still loved her, even after all these years. After all, she knew that if Elena was dead, then it had been quite a few years since she'd last been in this town.
"Well, truth be told, I still love you, as you still love me. I lied when I was a vampire. I was trying to protect you from anything Klaus might do to you. I was afraid back then that he'd kill you just for loving me, so I made you hate me, since I figured it was the only way to protect you. I knew Klaus was coming to town, and I wanted to protect you. You were always the one when it came between you two, just as I know you have always loved me the most, too."
She gave a smile.
He smiled back.
After a few minutes, it was such a moment, they began to make out. And so, their love story continued.
