Summary
Naruto has a twin sister. He really loves his sister because she is the only family he has. However, unlike him, his sister is weak. He is also aware of the relationship between them and the villagers. That's why he keeps his sister's existence a secret while he diverts all the attention of the villagers towards him by acting as a troublemaker. All of this is for the sole purpose of his sister's safety. For his sister, he's willing to do anything. Even if that means sacrificing himself.
Even from a young age, Naruto already notices the slightly different treatment given to him and his sister and to other kids in the orphanage. He always wonders why, but he ignored it. He has more important things to do, taking care of his sister. Unlike him, his sister Naruko is not as strong and healthy as him. Her skin is pale, her movement is slow, and his body is weak. That's why, even if he always wanted to confront their caretakers on their poor treatment towards them, he never did. He's scared that they might do something to Naruko if he openly confronts them. Even if he doesn't like them, they still need them to take care of them. He will hold back until he is capable enough to take care of both himself and his sister.
At least, that was the plan. However, real life doesn't always align with our plans. After returning to the orphanage from the Hokage office for some monthly allowance, he witnessed one caretaker attacking his sister. Seeing her injured, sacred, and crying was enough for him to lose his self-restraint. He screamed and jumped towards the caretaker. He tried everything, clawed her, bit her, punched her, absolutely everything a child could do to fight an adult. But, the power of a child could not match with the power of an adult. The caretaker managed to grab him and threw him to the wall. Feeling dizzy after he hit his head after he got thrown by the caretaker, Naruto lay for a bit, unresponsive. But the cry for help from his sister immediately woke him up. And in his rage, he grabbed a pair of scissors and charged towards the caretaker. When the caretaker is within his reach, he stabbed her in the leg. The caretaker then screamed in pain, but he was still not done. He stabbed her again in the back of her neck. The caretaker then fell to the floor. Seeing his enemy down, Naruto, blinded by rage, then stabbed her again and again. Blood splattered in all directions before he stopped.
All this commotion invited the other caretakers to come to their location. When they arrived, they were shocked by the gruesome sight that greeted them. They immediately secure the twins in a solitary room and call the Uchiha Police. The police force then arrived, however, they knew the case is very difficult because they are dealing with children and on top of that, a jinchuuriki. Thus, they passed the case to the ANBU. After the ANBU were called, they immediately informed the Third Hokage of this incident. Hearing this, The Hokage took off from his seat and went to the orphanage. He began questioning every single caretaker there. He even tried to intimidate and threaten them into talking, but none can give a single piece of information. All they know is they found a bloodied form of Naruto on top of a lifeless corpse of a caretaker he just killed. After confirming they are telling the truth, he then decided to ask the assailant himself. He entered the room that held both Naruto and Naruko. He could see that the expression Naruko had was one of deep fear. To be expected for a child to be traumatized after witnessing a murder. But Naruto, he seemed calm. But he knew better. Deep down, he could feel that Naruto was seething. The Hokage then asked him about the accident. Naruto told him. All of it. Without leaving any details. From start to finish. The Hokage, after years and years of experience in politics, can determine when someone is lying or not. And he deemed that what Naruto just told him was the truth. He even asked Naruko and she confirmed it. The Hokage then decided to close this case and made it a secret. After that, he and the ANBU left the orphanage.
However, people simply wouldn't forget. Even though they were sworn to silence, the other caretakers treat them even worse. They now treat them as a criminal on top of something Naruto had yet known. The victim of all, none other than his sister. Now they know that Naruto can fight back, brutally even. But his sister cannot. That's why they redirected all their hatred towards Naruko. They don't know that the reason that Naruto murdered the other caretaker is because she was assaulting his sister. This is because The Hokage kept the details of the incident a secret. Only a handful of people knew. Naruto, after hearing the treatment those caretakers did towards his sister, then attack them. This time, no one died. But they need to be hospitalized. The Hokage then intervened again to overcome this problem. When he heard that this case was due to the same causes as the previous case, he decided that the orphanage was not safe for this pair of siblings. He, from his own pocket, bought an apartment building. Then he ordered this pair of brothers and sisters to move into the apartment. He hopes that this way, there will be no more cases like this happening again.
Naruto himself have no problem with the plans made by The Hokage for their safety. However, after the second incident that happened to his younger sister, Naruto became paranoid. He still felt that there were still people after them, especially his sister. Thus, he made a request to The Hokage. The Hokage, after hearing his request, was very surprised. Naruto requested for the existence of his sister to be made secret. He was very aware that the hate towards them is not limited to the caretakers of the orphanage, but also from the other villagers. The Hokage was very sad to hear this, but he granted his request. From this moment on, there is no longer someone named Naruko. He then undertook a series of erasure attempts to achieve this. Then after months, it's done. Naruto, very satisfied with this, hoped that he finally can bring peace and happiness to his sister.

Finally, after a long wait of several years, Naruto finally becomes a ninja. After he was old enough, he was put into the ninja academy by The Hokage. The Hokage hoped that Naruto can open up and socialize with people of his own age. He also hoped that Naruto can reduce his paranoia a little. For Naruto, however, entering the ninja academy would allow him to become stronger so he could protect his sister. His time at the academy was quite monotonous. He considered that the curriculum was quite easy. But to keep things low-key, he held himself back. This is what later made him graduate from the academy as a mediocre student. In fact, he even thought of becoming a dead last student, but this attempt was thwarted by The Hokage once he heard it.
Over the past few years, Naruto has built an image of himself as cheerful, a troublemaker, and a bit of a dork. He even often makes pranks throughout the city. Annoying a good number of both villagers and shinobi inside the village. He did this not because he wanted to, but because he had to. By doing this, Naruto would then turn all the attention and hatred of the entire village onto him alone. Thus, his sister was spared. So far, his efforts have been successful. His sister can freely go around town without worrying about being targeted. His pranks were also seen as entertainment by his sister, so he still did this anyway.
Talking about his sister, at first he strictly forbade his sister to leave the house. He was still afraid that something might happen to her. But after being told by The Hokage that staying at home all the time is not healthy, especially considering the condition of his younger sister who needs fresh air from time to time, he gave in and let her out for some time. This was also caused by another factor, namely seeing his younger sister sulking and not wanting to talk to him. However, due to his paranoia, he still feels anxious and restless whenever she goes outside. He also couldn't openly accompany her because Naruko would most likely be targeted if associated with him. The Hokage who knows this then deploys his personal bodyguard from the Sarutobi clan to guard Naruko in secret every time she goes out of the house. He even made his sister cover her head or wear a wig whenever she went out so that she very distinguish blonde hair could be hidden. Although still paranoid, this measure has at least reduced his worry.
Now, he has been a ninja for about a month. His team is Team 7 consisting of Hatake Kakashi, as squad leader. A former member of the ANBU and also one of the strongest Jounins in the village; Uchiha Sasuke, the second son of Uchiha Fugaku, the leader Uchiha Police force; and lastly Haruno Sakura, the daughter of a villager. During his time as a ninja, he didn't go through enough. So far he and his team have only carried out D-rank missions. This is enough to irritate him. But he endured it. Now though, it was different. They had just carried out what was, in his opinion, the most annoying mission of all time. Namely catching the cat belonging to the Daimyo's wife named Tora. Not only is this cat very agile, but he is also grumpy and easily scratches people who want to catch him. Also, this isn't the first time Team 7 has done this mission. In fact, this is the third time this week. And today is the second time Naruto has fallen victim to his claws. So saying that he is angry is an understatement.
"Team 7 reporting," said Kakashi once they entered the Hokage office.
"Ah, Team 7. Welcome back. I assumed the mission had been completed?" said The Hokage.
"Yes, Sandaime-sama. We were finally able to catch Tora." Answered Kakashi.
"That's good to hear! Now please give it to Madame Shinji over here." Said Hiruzen Sarutobi, The Third Hokage, while directing his hand towards the owner of the cat, Madame Shinji.
Naruto, who was given the task of holding the cat, then walked over to Madame Shinji and gave Tora to her.
"Ahh, Tora. You are finally home. Please don't run away again, my dear." She said while giving the damn cat a bone-crushing hug. Seeing the pained and annoyed expression of said cat is enough to make Naruto snicker in amusement.
"Hokage-dono, thank you for bringing back Tora." Said Madame Shinji.
"Please, Madame, Team 7 did all the work. They're the ones you should be thanking." Said Hiruzen.
"That's true," she then turned around and fixed his gaze on Team 7, "thank you Team 7, for bringing back Tora to me." Said Madame Shinji. She did not bow, but you can still feel that her gratitude is sincere.
"No need to thank us, Madame Shinji. We are only doing our job. That's what a ninja does." Kakashi responded.
"Well if you say so," she smiles, "well then, Hokage-dono, Team 7, please excuse me." And with that, she leaves the room.
"Good job, Team 7. Now onto the next mission. We have painting duty, or some villagers need help cutting their grass. Oh, we also have missions requested by the Inuzuka Clan to walk their dogs. Or-"
"With all due respect, Hokage-sama, we want real missions! Not some trivial jobs like chores!" Said Sasuke while gritting his teeth.
'Ah, he snapped' Hiruzen thought, amused.
"SASUKE! You cannot say that to Hokage-sama. Immediately apologize!" barked Iruka.
"It's fine, it's fine, Iruka-kun," said Hiruzen.
"But, Hokage-sama…"
"I said it's fine. In fact, I think it would be great if there were ninjas who were bold enough to speak their minds." Said Hiruzen, "So you want a real mission, huh."
He then speaks to Kakashi.
"Kakashi, are they ready?"
Kakashi did not immediately answer. He first looked at his genin students. The three of them looked at him with hopeful eyes. Kakashi chuckled at this. He then looked in the direction of The Hokage.
He finally answers, "Yes. I think they are ready." He could see their delighted reactions from the corner of his eye. Well, as long as they are happy.
"Very well," said Hiruzen.
"Wait a minute!" cut Iruka, "I still didn't agree to this!"
"Iruka-kun. Please realize that they are not your students anymore. They are now great shinobi of Konoha. You cannot always babysit them."
"But-"
"No buts. Also, saying things like that is disrespectful towards Kakashi as their Jounin-sensei and their direct superior."
"Yes, Hokage-sama. I apologize, Kakashi-san"
"No problem." Said Kakashi.
"Well, if there's no more disagreement, let's just call Tazuna-san in."
After he said that, the door opened. Then entered an old man who looked half drunk and had a bottle of sake in one of his hands.
"These guys are my escort? Doesn't look strong. Especially that blonde."
"Want to say that again to my face, punk?" said Naruto.
"Maa, maa, we don't attack our own client, Naruto. And Tazuna-san was it? It's unwise to antagonize the people that you rely your life on." Said Kakashi, trying to ease the situation.
"Hmph, my name is Tazuna. I'm a great builder. We will leave tomorrow morning." With that, Tazuna leaves the room.
"Well, you heard him." Said Hiruzen. "Team 7 dismissed."
After leaving the Hokage tower, Kakashi gave a briefing to Team 7.
"Go home and rest. Don't forget to pack for a week or two. We will meet tomorrow morning in the Southern Gate at 8 sharp. Understood?"
"Yes, sensei!"
"Good, and don't be late." Said Kakashi before he disappears using Shunshin
"That should be our words." Said Sakura, clearly annoyed.
"Well, see you tomorrow, guys." Said Naruto.
"Yeah, see you tomorrow"
"Hn"

Naruto walked to the apartment where he lived. Along the way, he noticed the unsavory stares from the villagers. He was used to this, but he still felt irritated. After all, it's not his fault that the Fourth Hokage decided to seal the Nine-tailed beast inside his gut. Well, never mind that. As long as he still has his sister, he will be alright. He was absolutely certain of that.
As he was walking through the alleys, he was suddenly pulled into one of them. At first, he was wary and prepared to fight back in self-defence. But after he saw who the culprit was, he lowered his guard. It was The Hokage himself.
"Hokage-sama? What are you doing in here?" asked Naruto.
"I'm just a clone. The real me is still inside the tower." Hiruzen answered. "Tell me, my boy, what's been bothering you?"
"I don't know what are you talking about Hokage-sama."
"You are still a hundred times younger to lie to me, Naruto-boy." Said Hiruzen, "After you found out that you were going outside the village, you looked even more agitated. Even though you're really good at hiding it. If it wasn't for me knowing you for so long, I could have missed that."
"Well, it's just… I'm just worried."
"About Naruko?"
"Yes"
"Worry not, my boy. I've told you, haven't I? I will send my own personal guards to protect her. I even went through all the trouble and use some members from Sarutobi Clan to do so because you didn't trust some of my ANBU."
"Well,"
*Sigh* "Listen, Naruto. You have to stop worrying. It's not good for you. Moreover, you will be doing a mission outside the village. It would be very dangerous if your mind was split in two like this. You lose focus a little bit, then your life will be at stake."
"…"
"Boy, you don't have to worry. Your sister is safe here. I will also always be in the village because I am the Hokage. In addition, several trusted ANBU has returned from their missions. I'll reconsider letting them take a few days off so they can be with your sister. How is it?"
"Yes, Hokage-sama. I understand. I also thank you for your help to me and my sister." After that, he bows to the Hokage and leaves.
"You are welcome, my boy, also thank you for holding the beast, and lastly I'm sorry." Muttered Hiruzen before he was dispelled.

Hearing this, Naruko rushed to the front door to greet his brother.
"Onii-chan, welcome back! Do you want to have dinner or take a bath first?" greets Naruko.
"Whoa, easy there. We don't want you to get too tired." Said Naruto. Due to her weak physical condition, she couldn't get too tired. If that happened, then she would collapse. Naruto didn't want that to happen again.
"Fine, fine." Said Naruko, "anyway, your answer?"
"A bath please" replied Naruto.
"All right, I've already prepared the hot water. You can enjoy it now."
"Thank you, Naruko, you are the best."
"Hehe, thanks. Also, I've prepared dinner. So hurry up and take a bath so we can have dinner together."
"All right, all right. Anyway, what's for dinner?" Naruto knew that he shouldn't be asking this question.
"Ramen!" She beamed.
"*sigh* I knew it, you ramen freak."
"You can't complain because I'm in charge of cooking dinner, beh," she said while sticking out her tongue.
He just shook his head with an amused expression at his sister's behavior. He then entered the bathroom. After he finished bathing and getting dressed, he joined his sister who was already waiting for him at the dining table.
"Hurry up, Onii-chan, before the ramen becomes cold!"
"Yeah, yeah." Naruto then sat down at the dining table.
*clap* "Itadakimasu" Both said in unison.
While they were enjoying their dinner, Naruko tried to make small talk.
"So, Onii-chan, how's today."
"Well, at first it was normal. We gathered at Training Ground 7. We were given brief instructions. Then a bit of practice before we finally went to the Hokage tower to get our mission. Unfortunately, we got Tora's mission and I became the victim today"
"Really? The second time this week?" You can see amusement in her eyes.
"Yes,m unfortunately," said Naruto "then we got ourselves a new mission. This time is C-rank. We will be going outside the village to escort a bridge builder to a country called Nami no Kuni."
"Wow, outside the village," Naruko said with twinkling eyes "must be very different from here in Konoha, huh?"
Her expression then fell.
"If only I wasn't weak and sickly. Maybe I can join Naruto Nii-chan to become a ninja. We could have been a great, cool duo that everyone respected." She said sadly.
Hearing this, Naruto also felt sad. But he couldn't let his little sister continue to feel sad. He needs to comfort her.
"Hey, listen. You may not be strong like me. But I'm sure you have other Strengths that I do not have. Cooking for example. Your cooking is always delicious. I can't cook at all. You remember the story where Kakashi-sensei gave a camping simulation exercise, right?"
"Yeah, the one where you get tasked with cooking dinner, but end up blowing up the entire food supply."
"Yes, and once I managed to make food that looked edible..."
"You made Kakashi-san vomit so many times, Sasuke-san had diarrhea for three days, and Sakura-san had to be hospitalized for five days."

Naruko then gives him a big hug.

They hugged for a few moments before they finally separated from each other.
"Now, as I said before, I have a mission outside the village. I won't be here for a week or two or so. Help your brother here and if you can don't get out of here, okay? If you want to leave, please use the bird message and notify the Hokage or the Sarutobi Clan. And don't forget your wig, or at least wear a hat or hoodie."
"Yes, Mom," Naruko said.
"Don't be like that. I'm doing this for your safety. Do you understand?"
"Yes, yes."
"Good. Now let us sleep. It's late and I need to be up early next morning."

When Naruko woke up, her brother has already left. Well, that's to be expected. She was not a morning person while Naruto always get up early to start his morning exercise. After stretching for a while, she got up from her bed and went straight to the bathroom. After brushing her teeth and washing her face, she rushed to make breakfast for herself. This morning was pretty quiet, there wasn't much activity outside the apartment where she and her brother lived. So, she decided to watch television to pass the time.
Without realizing it, it was already noon. Naruko was getting ready to cook lunch. After she cooked lunch, she placed it on the dining table. After saying 'itadakimasu', she then ate the food she had cooked. After finishing eating, she immediately cleared the dining table and cleaned the kitchen. After that was done, she then washed the dishes and various dirty kitchen utensils. Once done, Naruko then decided to read a book her brother had bought her while eating a snack. This is more or less her daily life. Very monotonous and has not changed at all for several years.
If someone asked Naruko a question, are you bored with your life now? Then she would answer yes without hesitation. However, she couldn't do anything about it. Due to her weak and sickly physique, she couldn't do much. If she was too tired, she would faint again like last time and need to be treated for several days in the hospital. If this happened, then her brother would be very worried and restless. Her brother would not be able to calm down and would even stay up all night looking after her. Naruko doesn't want that. The food at the hospital isn't good either, so she doesn't want to go back to that place.
However, she was also very bored. So she quickly wrote down a message on a small piece of paper, then put it in a small bag, attached it to the pigeon's leg and flew it. After a while, the bird returned with a new message from the Hokage-jiji. Naruko asked for permission to go outside for a while, and the Hokage-jiji gave permission. He will send some ANBU members for her security. After getting confirmation, Naruko then got ready to go outside. She prepared necessary things, such as a wallet and whistle to alert others just in case. In addition, she also did not forget to bring her medicines in case of emergency. After she fixed a brown wig on her head, she waited until she was given the green light to go out. Then she heard a knock on the door. Naruko opened the door and saw a familiar figure. With a feminine body and long purple hair, Naruko knew that she was one of the ANBU that her brother and the Hokage-jiji trusted.
"Neko-nee!" exclaims Naruko.
"Naruko-chan, good afternoon." Greets Neko. "Ready to go out?"
"Yes!"
"Good. As usual, we will escort you while hiding. If you feel danger, just scream, or blow the whistle you brought, got it?"
"Got it."
"Excellent, you can go now. Please enjoy." After that, she disappears in a cloud of smoke.
After being given the green light, Naruko then went outside. Indeed, she still felt a bit awkward and a little insecure when she was in the outside world. However, she also felt that she was free and no burden when she went out. After all, she could see the state of the village she lived in.
Her journey begins by visiting a food stand. Even though ramen is her number one food, she still likes to try different kinds of food. After exploring and tasting the various food stalls, she went to a clothing store. After all, she is a girl who loves to look around and wear new cute clothes. After that, she took a walk in the park. Her journey today was quite entertaining for her. After the day became increasingly late, she was rushing to go home. But because she was in too much of a hurry, she didn't see where she was going and bumped into someone. Due to the significant difference in physical strength, Naruko fell backwards and landed on her back.
"Ouch..." she winced.
"Neji, look what you just did! You've just hit a girl!" says a girl with hair styled in two buns.
"That's right, Neji-kun, that is the most unyouthful thing to do!" shouts a guy in a green spandex.
"Be quiet, you two." Says the person who bumped into her, "I didn't even do anything. If anything, I'm the one who got hit, Tenten."
"But still, it's a girl that got hurt. You better take responsibility, Neji!" says Tenten, the girl with bun hair.
"…" her assailant (or victim), Neji, doesn't say anything and kneels in front of Naruko.
"Are you all right?" asks Neji.
"Yes, I'm okay," Naruko replies.
"Glad to hear it. Listen, I'm sorry, but please pay attention to your surroundings."
"Yes! Thank you, also I'm sorry." Naruko says. After that, she got up, gathered her scattered belongings, and hurried to her home.
"Weird girl, huh?" says Tenten after Naruko left.
"Yeah" replies Neji.
"Well, I hope we meet again, she's cute," says Rock Lee, the guy with green spandex.
Neji didn't know why, but he didn't like hearing those words from Lee.
