Luffy was dying. That simple fact was the only reason why the captain of the Heart-pirates were working to save him as frantically as they were, Trafalgar Law one of the super rookie pirates was the main doctor but he knew that only a miarlce would help in this predicament right here.

Luffy was confused yet calm in an completly unluffy-like way which was an awakening of his mothers side of his family which has a rather powerful background and only a few yet strong mebers, cutting out the Monkey d side of the family would be just as foolish with people like Garp the hero or Dragon the leader of the revolutionary army. so yeah it wouldn't be that much of a stretch to assume Luffy couldn't return to the Land of the Living however before that happened he had become psychologically stable after rethinking everything and he came to the startling realization that something or rather someone must change,which was obviously him and the first change will be his now smarter mind and overall perception of all, meaning he would start listening to that strange voice he heard often as a child which he later tooned out same with his ability to knock out people maybe he'd ask somebody for some very much needed help to better his... well everything from attitude to fighting style.
and that is how he awekened a force so powerfull that even the approaching former vice captain and first mate of the Roger Pirates Sivers Rayleigh had to use his own strong Haoshoku to keep on going and get a shiver of anticipation throughout his entire body at the presence he felt die shortly returning and becoming all the stronger for it.
