"Only three more appointments left before my day is done. Every day I question this damnable false life I am living," he thought as he entered the examination room.
Sitting inside there was some green-haired brat and his mother that he had to break some bad news to. Kyudai Garaki, or rather his current alias Dr. Tsubasa, sat down in front of the pair.
"Sorry kid, it's not going to happen" he grumbled out, the mother asking questions and forcing him to give a lengthy spiel of the signs of quirklessness.
The kid looked utterly heartbroken as the mother ushered him out of his seat, it seemed like he dreamed of becoming a hero just like every other four-year-old these days. "Certainly adding his name to the list of vulnerable potential subjects, see if we can't find some use for a quirkless in the Nomu program," he thought as they left the room, leaving him alone with his thoughts. "Looking at his records, he goes to the same school as my shitty grandson. It might be risky to break confidentiality and tell him about the kid's quirk status but…the more vulnerable someone is, the easier it is to manipulate them."

He saw the Midoriya kid a number of times over the years for miscellaneous checkups, each time noting how he always seemed to have cuts and bruises. "Bullying, eh? Tough luck kid, but all the better for us if an opportunity arises," Garaki mused. "From what we've gathered so far, the father works overseas and the mother is a nurse. Staging a disappearance shouldn't be too difficult under these circumstances, and no one would miss a quirkless brat. I shall decide his fate after whatever this appointment is for, Nomu or otherwise."
Once more, he entered the examination room and sat down. This time however there was a notable change, the brat had grown out his hair and was wearing feminine clothes. Peering down at the clipboard his nurse left behind, he discovered that the kid was apparently transgender and had changed his name.
"Now that's an opportunity on a silver platter if I have ever seen one," he thought. All For One had many quirks suited to changing a person's appearance, and "helping" someone in that regard was always an easy way to get them alone and unconscious for some…tinkering. He needed to discuss this further with his master, the kid may not be a Nomu candidate after all.
The appointment was brief, all he had to do was make the offer and the kid lit up like fireworks. He dismissed the pair and turned to his desk, flipping the switch underneath it to lock the room and block any potential listening devices.
It took a few further seconds to open the secure communications channel that linked him to his master. "Sir, I do believe I have found you a new asset…" he spoke into his monitor.
"Oh? What a pleasant surprise you have for me, Doctor. Tell me, what makes this one tick," his master's sickly deep voice rang out.
"One Midoriya 'Ayami', a transgender teen a few years younger than Tomura. He is quirkless, but still holds aspirations of becoming a hero." Garaki explained.
"Hmm, I see. Having a quirkless subject for the program would be…entertaining, but that heroic drive? We cannot simply waste this boy, not at this juncture. That blonde fool has taken down most of our double agents at the Commision and amongst the heroes," All For One replied.
"Indeed master, I suggest we provide this child with the body they want, and use the opportunity to leave a few extra…presents we can exploit," Garaki said, a disgusting smile rising on his face.
"Very well Doctor, I shall make myself available for this endeavor in the near future. These 'presents' shall be all the more…entertaining when they break UA's security like a children's toy, and even more so once we reclaim our agent from under their noses."
At that, the communication ends, and Garaki disables his security measures, returning to his duties as a simple doctor.

By the time the child's procedure rolled around, his primary workplace in Jaku City was secured from any unwanted eyes and ears.
"Nurse Mikage, please go retrieve the subject. Take them to the room next to the service elevator, we will handle the transportation from there," Garaki said as he went through a mental checklist for the umpteenth time. All For One was a patient man but he couldn't risk testing that patience.
Mikage was an excellent hire referred to him by their information broker, not only did she already possess all the necessary training and documentation, her quirk makes it exceptionally easy to misdirect and trick her targets. "Obfuscation" allows her to blur the details of herself and her surroundings, making people more likely to forget or misremember, or outright ignore blatant red flags provided she focuses on them.
In this case, the young Midoriya wouldn't even question being led through the maintenance hallways, the lack of other patients, and any details about the nurse or her actions.
Moments after finishing all necessary last-minute preparations, a swirling black mist began to pool across the room. All For One stepped through the warp gate, followed closely behind by his crown jewel of the Nomu project Kurogiri. His master was impeccably dressed in a dark suit as always, his white hair providing a stark contrast to the dimly lit room around them.
"I trust that our little experiment is awaiting us upstairs?"
"Yes master, Mikage was sent to fetch him a little while ago and-" the small chime of a PDA interrupted Garaki from within his coat pocket "It seems our subject is in place and ready for Kurogiri." he finished.
All For One snapped his fingers, and Kurogiri stiffened, a portal opening in the center of the room and depositing the unconscious Ayami, exam bed and all.
"Truly a rare specimen these days, it feels like it was just yesterday that the quirkless outnumbered us, still holding onto their faltering beliefs of superiority in this world," All For One spoke, his words holding no small amount of disdain. "And yet this child shall remain ignorant to the gifts I shall bestow upon it. Doctor, this one has a higher calling than to be a Nomu, be prepared to act should it react poorly."

He placed a hand on the forehead of the unconscious form before him, calling upon the first of numerous quirks he would need. "Be grateful child, you will get to live out your dream of heroism…for a short time," All for One thought.

The child had now given its end of the bargain it never agreed to, as such it was to be rewarded.
True Form. A quirk that was far more useful than he thought when he first plucked it over a hundred years ago. Despite having no personal use for the power, being able to fix people's inadequacies made it all too easy to use them however he pleased. Ayami's body shuddered as the quirk began taking effect, shaking violently as the very essence of her body was made anew.
"It would be a shame if you were unable to worm your way into a hero school with how weak and powerless you are, what kind of benefactor would I be if I let that happen, hm?" he whispered into her ear.
Mental Fortitude.
She winced at the transfer, as the pain of her genetic code being forcefully rewritten and her mind becoming a veritable fortress overwhelmed the effects of the anesthesia.
Tactile Psychokinesis.
Her body seized briefly at the introduction of a second quirk, before settling into fitful tossing and turning.
Vector Manipulation.
A trickle of blood began to flow from her nose and her skin paled, the Doctor hastily waddled over and prepared a syringe filled with a dark viscous fluid. As another seizure gripped her body, All For One placed a hand over her face once more, using a quirk to paralyze her entire body and allowing Garaki the opportunity to safely inject the liquid into her arm. A tense moment passed as the dark fluid flowed visibly through her veins, her heart rate spiking uncontrollably. Her body fell still, and the monitoring device blared with a flatline for a handful of seconds before coming back, her vitals returning to stable levels.
"You have certainly outdone yourself Doctor, a subject fit for three quirks with no alterations. Let it not be said the quirkless are useless, after all, they are rather…malleable."
