One minute Dean was relaxing on the porch and the next he felt a pop, then pressure followed by a trickle of fluid running down his thighs. He took a nose dive off the rocker when he tried to stand up and landed on hands and knees. That was when he realized his water broke and the twins wanted out of there. In just seconds of calling for Sam, his brother had him standing.
"Dean, tell me what's happening."
Dean, trying not to freak out and in turn freak Sam out so he kept his voice steady, "My water broke and I can feel contractions but they aren't bad yet."
Sam brushed his hair off his face and looked around, "Ok, let's get you to bed."
"No way, I'm not ruining our new mattress. Just…just some blankets, a couple pillows and I'll be fine. Besides, I don't think laying around is gonna get these babies out."
"Dean, no."
"Sam, yes." Dean put both hands on the porch railing and leaned forward, his voice cracked when he spoke and Sam could just feel the nervous energy rolling off his brother. "Sammy, don't argue with me. While you're in there, make me a PBJ and bring a bottle of water."
"Do you feel like we can make it back to Bobby's in time if we leave right now?"
Another contraction hit him, "No way. I don't want to end up giving birth in the truck and an infected or something worse shows up. Babe, get the stuff and let's do the breathing exercises. It's going to be ok."
Dean could hear him fumbling around in the cabin and when his brother appeared with the sandwich and water, Sam was coated in sweat. In the blink of an eye, Dean was on blankets and pillows, their birthing bag was there and he was eating a PBJ and washing it down with cold water from the cooler.
They waited it out together and at one point, Dean got up walked around for a while then tested out different positions like squatting but he was too front heavy, he tried hugging a tree but that didn't work either. The mom to be gave up and went back to the porch nest where Sam acted as his back rest and they did the breathing exercises together.
As the time drew near, Sam tied back Dean's male parts with a strip of soft t shirt just as they'd planned. The alpha was quietly concerned, wondering how tiny humans were going to come out of an opening that looked so small. He'd read up on omegas and how their unique anatomy aided in birthing but that didn't stop Sam from worrying. As time ticked on, he played a mixed tape of soft rock for Dean. The omega tapped his fingers, did a little singing and the music helped him concentrate and relax at the same time.
The weather certainly cooperated for the Winchesters with sunshine and a pleasant summer breeze which created a natural symphony of creaking branches and rustling leaves throughout the surrounding woods. It was as if the heavens were welcoming in the new arrivals to this big blue marble and the angels knew just how important this ragtag little family really was to humanity.
As the time passed, Dean never once showed how exhausted he really felt. This was mostly for Sam but partly for himself. A Winchester never gives up and keeps going no matter the situation. He spent the time doing his breathing and "talking" to the kids in his head. Over the months he'd found this very calming and wondered if they understood any of it but probably not.
"Come on kids…I'm evicting you…the rents due and you don't have the cash to stay in my overcrowded luxury apartment…It's not so bad out here and there's a big family waiting for you…well not that big but we make up for it in ways…you have an awesome grandpa and I guess a sort of grandma with a cute butt and red hair…an uncle the size of a tea cup poodle and another uncle the size of a Mack truck and he's a principle so he and your smarty pants dad can educate you…better not talk about school or you'll never come out…Oh yeah, we've got chickens and a cat called Ginger…Your dad swears one time he saw a Big Foot in the salvage yard and he left it biscuits but he was still a kid and your dad had a big imagination back then…"
Sam rubbed his brother's belly gently in hopes it encouraged the babies to come quicker. Every time a contraction hit, Dean's face would scrunch up but he did not make any noise, just rested on Sam with his eyes closed thinking about something. Sam guessed he was having one of his mental talks with the kids.
They got their wish just an hour later when Dean felt the first baby coming, he switched positions to find one that that worked and ending up griping the railing, on one knee and the other knee bent. He let out primal, guttural sounds as Sam knelt behind him, ready to catch the first baby. Dean let out a grunt and their son's head popped through the stretched lips before finally sliding from the birth canal. Sam checked the infant boy's airway and then rubbed him down with a soft towel. The infant let out a wail in protest of being pushed out of his cozy home that quite frankly, had been pretty cramped the last month.
Sam clamped and cut the umbilical cord and wrapped Nicholas Robert Winchester in a fresh towel and showed him to his mother. He gushed, "Dean, he's beautiful."
Dean, his head resting on the railing and face covered in sweat, smiled and the squalling bundle, "Hi Nick, welcome to the big world." The infant stopped his crying to listen to the voice he'd heard for 9 months. Dean had all of two minutes to turn and admire the squishy faced little bundle kicking up a fuss before he felt the other baby coming quick.
"Sam, she's coming."
Sam carefully placed their son on a blanket, got in position and with his big, catcher's mitt hands, guided their daughter, Emma James Winchester into the world.
Dean dropped his shaking knee as soon as Sam clamped and cut the cord. The omega was exhausted but he didn't hide it anymore. The new father eased him back on the pillows after checking over Emma and rubbing her gently with a towel. Her cry was high and reedy but strong. Though smaller than her brother, she was healthy and pink. Sam propped Dean against him and the twins rested on their mother's chest. The afterbirth was expelled around thirty minutes later.
The sun was low in the sky by the time it was all over. Sam brought the infants inside before helping his tired and sore omega. He tended to Dean who was now on their comfortable bed, feeding both babies. There was no learning curve for either twin, they latched on right away and drank hungrily. The new parents marveled at how delicate and helpless they were. There was no way to tell who either of them looked like, they were wrinkled and pink but after the journey they just took, it was perfectly normal. Sam and Dean thought they were perfect and the most beautiful babies ever born in the entire world history of human babies.
Nicholas wrapped his tiny fingers tight around Dean's pinky and looked up at his mother. When he saw the omegas big smile and the happy crinkles appear at the corners of his green eyes, Nick made a little O shape with his lips, completely mesmerized. Emma had her eyes focused on her father, watching Sam's every movement. Both were bald except for a wisp of blonde hair on top their heads, their eyes were blue but as with most babies, eye and hair color could change drastically as they matured.
When the twins were done nursing, Sam burped them under the watchful eye of his brother. Dean thought it best to let Sam get the hang of things right away plus Dean just passed a couple bowling balls through his chute and did not feel like doing much of anything. It was sweet watching his brother fumble with the tiny diapers and little sleep sacks with his big hands.
Sam left and came back with a bucket of warm, soapy water and a wash cloth for Dean. His brother complained rather weaky while his brother ran the cloth between the standing omega's legs. Dean felt bruised and swollen and having someone wash him like a baby was frankly embarrassing even if it was Sam. Things got worse when Sam held up a postpartum pull up.
"No way, I'm not wearing a diaper."
"For someone who wanted to give birth on the porch because of the mess, your sure complaining…sorry, I mean you know there's going to be leakage for awhile and these will help. Look, its mesh with a long pad inside just for omegas."
Dean let out a sigh of resignation, knowing Sam was right. He pulled them up and took Sam's hand to take a short walk inside the cabin to keep his blood moving.
That night, Sam checked on Emma and Nicholas around a dozen times. They woke up around midnight, both crying. Sam checked their diapers before positioning them on their mother's chest so they could feed. Dean fell asleep half way through, confident Sam would take care of the rest.

The new parents only stayed at the cabin a few days after that. It became too hot inside even with the windows open and that issue was put on a long list of problems to solve. Dean longed for the closeness and safety of family and so did Sam. The new dad was overwhelmed and needed home for the same reasons as Dean. The infants rode in the tiny back seat area, both strapped in car seats that engulfed their little bodies. On the drive back, Sam could tell his brother was uncomfortable and sitting that long was giving him pain.
When Sam pulled through the gates of Singer Salvage, it was a huge relief.

Alfie shouted excitedly, "Cay, Bobby, they're back and…oh wow…Sam has a baby carry thingy in each hand!"
