I have seen multiple stories that have a reconciliation between Derek and Addison because Addison gets hurt; however, I don't recall reading one where Derek getting hurt. This story is very much AU, in that Derek and Addison have been married longer, have 3 kids, and it is set more in the present day. Also, Addison didn't stay with Mark after the affair. Addison sat in the private jet fidgeting with her jewelry and swinging her foot. She looked around at her children and was very thankful at least they were sleeping.
Eleven year old Apollos was sprawled across the front. He was her quiet, gentle one. He bore a striking resemblance to the Captain with his sandy blonde hair, but his eyes were all Derek. It tore at Addison's heart at how quickly her oldest son was growing up.
Eight year old Khaliel was curled up to the seat on her left. He was her crazy, outgoing son. He was a mini-Derek with dark brown hair, except the eyes. He had her green eyes. Addison smiled as she looked down at her sleeping son and moved a curl out of his face.
Six year old Elliette was curled up on Addison's right side. The only girl in a family of boys. She was very tiny for her age, so much so that they had to have extensive testing done when she was a toddler to make sure everything was ok. She just was small, like her Aunt Amy. Elliette had dark red hair like Addison with curls from her father. She also had the piercing blue Shepherd eyes. Heaven help them when she was a teenager!
Addison's mind wandered from her children to the call she had received less than 6 hours ago. Had it really only been 6 hours?!
"Addison, you need to come. Derek has been in an accident." is what the voicemail from Richard had said. Before that call, she didn't even know where Derek was! Seattle. He had driven across the country and left his family behind. Of course, she couldn't exactly blame him. He DID walk in on Mark and her having sex. She hadn't heard from him in 6 weeks.
Ugh! When did her life become so messed up? Addison quietly got up from her seat and walked around the plane. She didn't want to wake the children, but she also couldn't sit still.
"Derek has a severely broken ankle, a broken tibia, several broken vertebrae in the lumbar section, and a brain bleed. He is being taken into surgery now." That was the last text she had received from Richard 2 hours ago before they boarded the plain. No reason had been given as to what had actually happened.
Addison returned to her chair. She had never been more thankful to have a family private jet than she was right now. There was no way they would have gotten on a plane so quickly otherwise. As it was, she had barely packed anything.
Six hours earlier
After scrubbing out, Addison checked her phone as usual. She had a voicemail from Richard Webber. She rolled her eyes. Her former mentor had been trying to get the Shepherd's to move out to Seattle for years to work at his hospital.
When she heard the voicemail, all time stood still. Derek had been in an accident. She had no idea what had happened or what the extent of his injuries were. She tried calling Richard, but of course, he didn't answer.
Addison stood frozen in the hallway of Mt. Sinai for a few minutes. What was she supposed to tell her kids? Would Derek survive?
"Addison, are you ok?" her friend Casey asked as she walked up to her. "You look like you've seen a ghost." she joked. Addison looked up at her, fighting back tears.
"Oh my gosh, Addie! What happened?" Casey asked. She had never seen her friend cry before!
"Derek was in an accident." Addison choked out. Casey was shocked.
"Is he ok? Where is he?" Casey asked. She was one of the few people who knew about the affair and that Derek had left town.
"He is in Seattle. I, uh. I don't know if he is ok." Addison replied. She looked at the horrified look on her friend's face and knew it would be much worse with the kids. She had to pull it together. Addison shook her head to snap out of her shocked state and flew into action.
"I have to go, Casey. I will update you as soon as I know something." Addison replied. She turned and left before Casey could respond.
Immediately, Addison called the Montgomery private jet company to book the plane. They could have the plane ready in 3 hours. Next, Addison went to Chief Rhenholds and told him briefly what was going on. Of course, he told her to take as much time as she needed. Finally, it was time to go get her children.
Addison walked into the Brownstone and was immediately tackled by Khaliel.
"Mommmmmmmmmmmyyyyyyyyyyyyyyyyyyyyyy!" he yelled as he ran across the floor. "You are home early!" he said with glee as she gave him a hug.
"Mommy, guess what I learned in gymnastics today?" Elliette said excitedly as she got up from the couch. "I got my back handspring all by myself!" she answered before Addison had time to respond. Elliette, or Ellie as they called her for short, had just been moved up to the team at her gym. It meant long hours, but the young child loved it.
"Wow, Ellie! That is great!" Addison replied giving her daughter a hug. "Shannon, could I talk to you in the kitchen please?" Addison asked the nanny.
"Sure." Shannon said, confused. Addison rarely requested to speak to her alone.
"Oh my gosh!" Shannon exclaimed once Addison told her about Derek. "Are you ok?"
Addison smiled at the young college student who was watching her children this summer. The poor girl had been roped into way more than anyone expected this summer with the affair, Derek leaving, and now this.
"I am ok." Addison assured her, even if it wasn't the whole truth. "I don't want to tell Khaliel and Ellie as they will be too upset. Could you go pack some clothes for them while I talk to Apollos?"
"Of course, Dr. Shepherd. Whatever you need!" Shannon exclaimed, exiting the kitchen to go help with the packing and to distract her younger to charges.
Addison quickly made her way up to her oldest's bedroom. He, as usual, was drawing. It always blew Addison away how good of an artist he was.
"Hey, Mom." Apollos smiled up at her as she walked in. His smile immediately went away when he saw her face. "What happened?" he said looking at her seriously.
"There's been an accident, Apollos." she said tentatively. She knew that sugar coating things for her oldest never went over well. He was such an old soul that they had always told him things straight out.
"It's Dad, isn't it." he said angrily.
"Yes. I don't know much. I just know that we have to go to Seattle." Addison answered. Apollos immediately threw his pencil down and stood up.
"This is your fault! If you hadn't cheated on him, he wouldn't have left!" Apollos spat. Addison felt like she had been slapped across the face. Things had been rough with her 11 year old since Derek left. He of all the children had taken it the hardest.
"I need you to pack some clothes and your backpack with whatever you want to bring." Addison said as calmly as she could. "We are leaving for the airport in 30 minutes." She got up and left the room. She knew her son would get ready but didn't know what else to say to him.
Addison quickly made her way to her bedroom where she collapsed on a chair. She wanted to stay there and cry, but she knew if she started, it would be really hard to stop. Her children needed her to be strong. She took a deep breath and went to pack for herself. She had no idea what they would need or how long they would be gone, but whatever they didn't pack, they could always buy.
Soon all 4 Shepherd's were loading up into the town car that was driving them to the airport. Khaliel and Ellie were excited about a trip. Apollos wouldn't talk to anyone. It was on the way to the airport that Addiosn got the updated text from Richard with the list of Derek's injuries.
Addison just stared at her phone. What in the world had happened to Derek that he had so many broken bones?! And a brain bleed!? Surgery?
"What the hell happened?" Addison had texted back, but she received no response before they were airborne.
It was 6:00 Pacific time when they landed down in Seattle, which meant to their bodies it was 9:00. Addison had a slew of texts from Richard once they were back on the ground.
"I will explain everything when you get here."
"He is still in surgery but vitals are good."
"Take the kids to Adele."
The last text was a huge relief to Addison. She had no idea what she was walking into with Derek, and really didn't want the kids there.
"Mommy, is that the OTHER ocean?" Ellie asked with wide eyes as they crossed the ferry to Adele's house.
"It connects to the Pacific ocean." Khaliel answered her matter-of-factly. "But Ellie, there are more than just 2 oceans." he informed his sister. Addison smiled down at her youngest two. They still didn't know why they were truly in Seattle. Until she knew the full extent of Derek's injuries, she didn't want to say anything to them.
Apollos had stayed silent the entire time. He had always been close with his dad. Addison knew that the last 6 weeks had been really hard on him. When Addison had informed him that he was going to stay with Adele while she went to the hospital, Apollos had just stared at her.
"Adele, are you sure you will be ok with all 3 of them?" Addison asked worriedly. It had been a few years since they had seen Richard and Adele.
"Addison, we will be fine. Go!" Adele said, practically pushing Addison out the door.

Addison got out of the town car, thanked the driver, and looked up at Seattle Grace. It was so different from Mt. Sinai. It was more spread out and she even saw an outdoor eating place. She entered the lobby and went straight to the front desk.
"Where can I find Richard Weber?" she asked.
"Do you have an appointment with Chief Webber?" the lady at the desk asked.
"Not exactly, but he is expecting me." Addison responded.
"And you areā¦." the lady asked curiously.
"Dr. Addison Shepherd." she answered. The lady looked at her shocked.
"Shepherd?" the lady asked in surprise.
"Yes, My husband, Derek Shepherd, was in an accident. Please, mam, where is Richard Webber." Addison said, beginning to lose her patience.
"Oh. Um, I am not sure exactly, but Richard is probably up in the surgical wing." the lady said quickly.
Addison followed the directions to the surgical wing and immediately approached the nurses station when she didn't see Richard.
"Where is Dr. Webber?" she asked.
"I believe he is monitoring a surgery. Who are you?" asked nurse Debbie.
"I am Dr. Addison Shepherd. My husband, Derek, was in an accident. I need to find Dr. Webber." Addison was about to lose it. All she wanted to know was if Derek was ok.
"Shepherd?" nurse Debbie asked, raising an eyebrow.
"Yes, Dr. Shepherd." Addison said again. She noticed a few other people around the nurses station staring at her. Why was everyone reacting this way to her?
"Addie, you are here!" Richard exclaimed as he made his way around the corner.
"Richard, thank goodness!" she replied, giving her mentor a hug. "Where is Derek?"
"He is still in surgery."Richard answered. He noticed they were starting to draw a crowd. "Let's go talk in my office." he continued as he guided Addison down the hall.
"What the hell happened, RIchard?" Addison demanded as soon as they were in his office.
"Derek fell 19 feet off of a roof." Richard said as he gestured for Addisoon to sit down.
"He fell off of a roof?" Addison asked in confusion. "What the hell was he doing up on a roof?"
"He was helping Meredith fix some shingles on her house." Richard said nervously. Addison looked at him more confused than ever.
"Meredith? Who is that?" Addison asked. Richard didn't say anything for a few minutes and then it clicked. "It is his girlfriend, isn't it." she said, slumping back in her chair in defeat.
"Meredith Grey is an intern here at Seattle Grace, and yes, it appears that Derek and her had some kind of relationship." Richard said awkwardly. "Addison, I don't know what happened between Derek and you or even what is going on between Meredith and him. You will have to talk to him about that."
Just then, Richard's phone rang. "Derek is out of surgery." he told Addison as he put the phone down. Richard got up and Addison followed him out of the office to recovery.
As Addison entered the recovery section of the surgical floor, she was unaware of a group of interns who were standing around watching.
"Who is that with Dr. Webber?" George asked Izzie. "She is hot!"
"I don't know." Izzie said, rolling her eyes. "I heard one of the nurses say she was asking about Derek. Maybe it is one of his sisters. Meredith said he had a lot of them." she shrugged.
"How is Meredith doing?" George asked out of concern. He had been at the house when Derek had fallen off the roof. Meredith had been eerily calm through it all.
"I think she went to go get something to eat. The Chief made it perfectly clear that we are to stay far away from Derek until further notice, but he wouldn't say why." Izzie said as she continued to watch Dr. Webber and the redhead. Richard shot both George and her a look when he caught them watching them, causing the two interns to disperse quickly.
Addison stopped dead in her tracks when she saw Derek. He was lying in the bed with casts on his leg and ankle. His head had been shaved and was wrapped in a bandage. He was still unconscious. It took everything in her not to cry out at the look of her injured husband.
"I want to see his chart." Addison choked out as she walked over to the bed. She took Derek's limp hand and realized how luck he was that both of his hands seemed unscathed by the accident.
"I have instructed everyone to leave Derek alone outside of the doctors who operated and need to check on him. I will give you some privacy." Richard said quietly as he handed Addison the chart. He squeezed Addison's shoulder in support and left the room.
"Derek, what have you done?" Addison whispered as she sat down in the chair next to the bed. She opened the chart and began looking through it. It didn't really tell her anymore than she already knew. The brain bleed had been resolved and barring any complications, it shouldn't have any long term effects. His tibia would heal with time. Thankfully his broken vertebrae did not injure the spinal cord. There really wasn't anything they could do but let it heal. Same with his coccyx. Those would be an extremely painful recovery, Addison thought. And they could cause issues down the road with pain, BUT, he wasn't paralyzed, so that was something. His ankle is what Addison was concerned about. The break was really bad.
Just then another doctor walked in.
"Oh, hi. I am Dr. Torres, the orthopedic surgeon. Who are you?" Callie asked, confused why there was someone else in the recovery room with Derek.
"I am Dr. Addison Shepherd. Derek's wife." Addison answered, standing up to shake Callie's hand. She didn't miss the look of surprise that flashed across Callie's face.
"His wife? Of course." Callie said quickly, trying to recover from her shock. She had no idea that Derek even had a wife. "And you said you are a doctor?" she asked curiously.
"Yes, I am a neonatal surgeon with board certifications in both Obstetrics and Gynaecology and Maternal and Fetal Medicine." Addison rattled off.
"Wow!" Callie said, again surprised.
"I was looking through my husband's chart and had questions on his ankle." Addison continued.
"Yes. It is going to need surgery, but it is too swollen right now to do so. I have put it in a cast for now and will re-evaluate it every few days." Callie informed Addison.
"What do you think his recovery time will be with all of his injuries?" Addison asked, glancing over at Derek.
"It will be several months at minimum but he should be able to leave the hospital in a few weeks." Callie answered. "He will need a lot of physical therapy and the recovery will be painful, as I am sure you assumed." Addison nodded in response.
"I am also assuming traveling to New York is out of the question for much of his recovery." Addison said, more to herself than to Callie.
"Yes. I would advise against any long distance travel for some time. Why New York?" she asked curiously.
"That is where our home is." Addison replied, looking up at Callie. "When will he be moved out of recovery into a private room?"
"That would be a question for Dr. Nelson. He is the neurosurgeon who operated on Dr. Shepherd's brain bleed. He should be by shortly." Callie said. "If you have no further questions on his breaks, I will leave you alone."
"Thank you." Addison said, giving Callie a slight smile. Addison returned to the chair and held Derek's hand. Callie watched them for a few moments before leaving.
"Derek has a wife. What other secrets is he keeping?: Callie thought as she exited the room.
As Callie pulled the currents back to give the Shepherd's some privacy, she was not surprised to see George, Izzie, Christina and Meredith had gathered again at the nurses station.
"What are the four of you doing here?" she asked. "Dr. Webber told everyone, especially you guys, to leave Derek alone."
"Who is that woman in there with him?" Christina asked.
"Why can't we go in there?" Izzie questioned.
"I just want to know if he is ok. " Meredith said sadly.
"He is doing fine. And who that woman is is none of your business. Now leave, or I will report you to the Chief." Callie said, raising her eyebrows until the interns dispersed. She didn't know what was going on between Derek and his wife or Derek and Meredith, but she DID know that now was not the time for Addison and Meredith to meet.
Dr. Nelson came by the recovery room ro check on Derek. Addison noticed that the man was actually about the same age as Derek and her and he seemed very competent.
"I am going to take Dr. Shepherd for a post surgical MRI. As long as everything comes back normal, there is no reason why he can't be moved to a private room." Dr. Nelson said quietly. Addison also noticed the man seemed a little intimidated by her. Or maybe it was the fact that Dr. Nelson had just operated on his boss.
"Thank you. I need to make a few phone calls, but please come tell me once Derek is out of the MRI." Addison said sweetly. She left the room and headed to RIchard's office. It was the only place she could think of to get some true privacy.
"Hey. How's he doing?" Richard asked as he looked up from his desk.
"Ok. He is getting a post surgical MRI right now. I have requested that they keep him sedated until the morning. His body needs some time to heal." she said tiredly.
"And you need time to sleep." Richard said with concern. "Let me take you back to my house, where you can get some proper sleep, Addison. I talked with Adele. The kids are already asleep. You can crash for a few hours at our house and then come back in the morning. If anything changes with Derek, I will be paged and will let you know immediately."
"I really need to call his family." she said looking down at her phone.
"Addison, it is 9:00 here, which means it is midnight in NY. You can call them in the morning." Richard said.
Addison mulled it over for a minute and then replied. "Ok. As soon as I know Derek is settled in a room, we can leave."
An hour later, Addison was finally alone in one of the Weber's guest rooms. She collapsed on the bed and began to cry. The stress of everything had finally caught up with her. How was she supposed to tell the kids what happened? Would Derek even want her around now that it appeared he had a girlfriend? How would the kids handle that? And where were they supposed to live as it looked like they would be in Seattle for a while, if Derek wanted them to be.
These were all the questions that were running through Addison's head as she eventually cried herself to sleep.

Addison woke up around 5:00 the next morning with gritty eyes from crying so much the night before. She groaned inwardly. She had hoped Derek's accident had been a really bad dream. She took it as a good sign that Richard had not woken her up in the middle of the night. She also knew it was time to call Derek's family.
"Carolyn, Derek has been in an accident. Please call me back. Or at least call Nancy." Addison said in a voicemail to her mother-in-law. She really hadn't expected her to pick up. Carolyn had refused to talk to Addison since the affair.
"Addison?" Nancy said, a little confused why her sister-in-law was calling her at 8 in the morning.
"Hey, Nancy." she said cautiously. While she was at least on speaking terms with her fellow OBGYN, their relationship had been strained over the last 6 weeks.
"What happened?" Nancy asked immediately picking up worry in Addison's voice.
"Derek has been in an accident." Addison said. She proceeded to tell Nancy the condensed version of what happened, leaving out whose roof Derek was fixing, and explained his injuries.
"Oh my gosh, Addie! What do you need?" Nancy exclaimed.
"Honestly, I am not sure. I haven't actually spoken to Derek yet. I haven't even told the kids as they are still sleeping. For now, I mainly just need for you to tell everyone. But I also don't think anyone should come out here yet." Addison added quickly. "At least not until I talk to Derek."
"Of course. I will let everyone know." Nancy answered.
"Thank you, Nancy. I will update you later today." Addison said as she got off the phone.
Addison got a shower and then made her way into the kitchen. Adele was making pancakes and coffee.
"Morning, Addie. How are you, dear?" Adele said with concern.
"I am doing ok. I will be better once I get some coffee." Addison laughed. "I need to get to the hospital soon. Are you sure you are ok watching the kids again?" Addison said as she toyed with her sleeve.
"Addison, I wouldn't have it any other way. Please don't worry about them" Adele said, putting her hands on her hips.
"Thank you. " Addison said as she sipped her coffee. "I will be back around lunch time to tell them what is going on. I am hoping Derek will be awake by then."
"Of course. Take as much time as you need." Adele replied.
Richard drove Addison to the hospital and showed her where Derek's room was. As expected, he was still sedated. Little had changed over night.
"I will be around if you need me, Addie." Richard said after looking over Derek's chart.
"Thanks, Richard." Addison said, giving him a small smile.
She walked over to Derek after Richard left and just looked down at him. If she blocked out the head bandage, he almost looked like he was just sleeping. While she was standing there, a group of doctors came in.
"We were going to do rounds now, if that is ok." Callie said when she saw Addison.
"Yes, please proceed." Addison said. She was eager for an update. She listened as one of the interns listed off the list of Derek's injuries and how they should proceed. She noticed that the interns kept giving her sideways glances.
"When are you going to lift the sedation?" Addison cut in.
"We will begin lifting it immediately, Dr. Shepherd." Dr. Nelson answered politely. "He should be awake within the hour." Addison gave him a slight nod and then walked back over to Derek's bedside, sat down, and took his hand.
Alex and George gave her an odd look as they exited.
"That is definitely NOT his sister." Alex said when they were out of the room. George didn't respond but was thinking the same thing.
Thirty minutes later Meredith made her way down to Derek's room. She knew she wasn't supposed to be there but she couldn't stay away. She wanted to know that he was okay. What she saw completely confused her. The woman with the red hair was back. She was holding Derek's hand and was sleeping with her head rested on the side of the bed.
It was then that Meredith realized that Derek was awake. She saw the look in his eyes when he saw the red headed woman, and then she saw him take his hand and rub the top of her head. Then Derek looked up and saw Meredith in the doorway. The anguish in his eyes told her all she needed to know. This was not his sister. It was his wife.
