Chapter 4: Twist
SCENE: ATTENDINGS LOUNGE
(Amber enters the lounge to find Jo alone in front of a tablet)
Amber: Morning I would include 'good' but you know.
Jo: Hey what are you doing here? I thought you were helping with Grey.
Amber: (Yawns) I am I worked the night shift and have been monitoring Grey outside her room. I need coffee and your lounge has the good pods. Who are you talking to?
Alex: (On the tablet) Hey kid.
(Amber leans behind Jo and grins at Alex on the other side of the video chat)
Amber: Hey, are you at the hotel?
Alex: No, I'm at Izzie's. I tested negative this morning and it's my day off so I thought I would spend the day with the kids. How's Meredith? How are her scans? Are there glass opacities?
Amber: Okay slow down please, I just left the floor ten minutes ago. Last I checked her sats were at 92% so we're putting her on six liters nasal cannula. We did scans last night and there are no glass opacities or rounded morphology not even lower distribution. Altman is on her case so you know that she has this Alex and will cover all the bases. Best case scenario she'll be kept under observation for a few days and discharged to a hotel where she can yell at the catering staff instead of the doctors on her case.
Alex: (Chuckles knowingly) Yeah Mer is a better doctor than patient I know that for sure. Tell Altman to call me, as her POA I should be updated with any and all changes.
Amber: She can try Alex but there are other things to do that-
Izzie: (Behind Alex) Hey are you talking to Jo and Amber?
(Izzie kneels down in full view causing Amber to frown at the sight of her)
Izzie: Morning guys.
Jo: (Octave higher) Hey Izzie how are things in the ER still overwhelmed?
(Amber stands up and goes to the Keurig with Izzie taking note of her leaving as does Jo)
Izzie: Yeah, but I'm helping out as much as I can so is Alex. It's not as overwhelming as taking over Mer's service, I'm sure.
Jo: Um yeah, I haven't seen the light in four days but Bailey asked me to and Mer has enough to worry about.
Izzie: How is she? Is she okay?
Amber: (Out of view with disdain her voice) As I told Alex her lungs looked good on the scans and we're keeping her on oxygen. Meredith is sure she just needs quarantine in a hotel after this.
Izzie: (Purses her lips) Sounds like you have things under control at Seattle.
Jo: Hey where are the twins?
Alex: They're at school upstairs, I think they like having classes online because they get to go to school in their pajamas. Although I hope this pandemic ends soon so all parents get their free eight hours back. And if Amber is still there, she should know that Alexis loves the book she got her and she can't wait for the second one.
Amber: (Grins and sips her coffee by the kitchen)
Jo: (Chuckles) Don't worry she's still here but I can't be I have to go to the ER for consults. I'll talk to you later, stay safe I love you.
Alex: Love you too, keep me updated on Mer, Amber!
Amber: Will do.
Izzie: Take care of her please.
Amber: (Flat tone) I always do with patients Stevens.
(Jo mouths sorry to Izzie who fumes a little but stands up as Alex ends the call. Jo gives Amber a scandalous look while she sips her coffee)
Amber: What?
Jo: Every time she comes to the chat you immediately walk away like she's Medusa on a screen.
Amber: Would you rather I insult her to her digital face? Besides you're just as bothered by her presence as me only difference is your classier than I am.
Jo: I am not bothered by her, what would make you think that?
Amber: (Mocks Jo's high voice) 'Hi Izzie how are you and my husband that is at your house doing?'
Jo: (Laughs) I do not sound like that.
Amber: Yes, you do, your voice goes up an octave when you feel awkward and seeing your husband's ex-wife brings out the squeaky voice. (Groans) I can't imagine how you're gonna be when you actually meet her in person. God knows I froze when I saw her bleached head for the first time.
Jo: And then you unfroze and punched your brother in the face.
Amber: (Laughs at the memory) That's right I did, hopefully you do the same only it's Stevens on the other side of your fist.
Jo: Man, you really hate her, don't you?
Amber: And why shouldn't I? She abandoned Alex with a letter and half a million-dollar medical bill. She then used the embryos he provided without so much as a text telling him she was using them. She kept him out of his kids lives for five years. It's like she made it her life's mission to make everyone hate her and she is accomplishing that.
Jo: Alex doesn't hate her.
Amber: My brother has been known to have piss poor judgement of people especially with the ones he slept with.
Jo: (Raises an eyebrow)
Amber: You're the exception, you're an example of him not being a complete dumbass.
Jo: I'm honored? I don't time for this I need to go to the pit; we can continue you ranting about Stevens tonight.
Amber: Better clear your schedule for tomorrow then.
Jo: (Chuckles and puts on her mask) Update me and Alex on Grey, I'll see you tonight.
(Jo leaves the lounge and Amber continues drinking her coffee)
LATER AT THE COVID WARD
(Amber sighs and looks at her watch waiting for Teddy with Andrew as well)
Andrew: Have somewhere you need to be?
Amber: No, now that Webber forbid the residents from personally treating covid patients my schedule cleared up a bit.
Andrew: Thank God for that, now you have to limit your exposure.
Amber: Yeah, I'm sure that Alex will be pleased to hear that when he wants an update on Grey. If he gives me hell, I'll tell him Fox replaced Koracick with someone who is competent and has a heart. And a keen understanding in respect towards women.
Andrew: (Chuckles) Yeah, I hear Webber is punishing him by making him teach the new interns, I'm sure that's good news to your ears.
Amber: (Exhales in joy) So much.
Andrew: (Looks to their right) Heads up.
(Amber sees Teddy walking toward them)
Teddy: Sorry, I went to get a few hours of sleep.
Andrew: Hey. Okay. Present.
Amber: Yep, patient was admitted for respiratory support four days ago, after a positive Covid test. Her O2 is 92% on six liters nasal cannula, WBC count is downtrending, and her latest creatinine is .89.
Teddy: How's she feeling? Any body aches? Increased fatigue?
Meredith: (From her room) Why don't you ask me yourself?!
(They look at Meredith in bed on her tablet)
Andrew: Lung's sound good.
Amber: Yeah, and I thought she was loud yesterday.
(Teddy enters the room leaving Amber and Andrew outside)
Amber: Do we have her latest scans? Are her lungs still clear of opacities?
Andrew: She refused scans actually and tests.
Amber: (Shocked) What? Why the hell would she do that?
Andrew: Because she's more stubborn than any of us?
Amber: (Groans) Well I'm scheduling her for a chest CT anyway, if there's one thing we know about covid is that it can strike in unexpected ways.
Andrew: And you don't want to piss off your brother.
Amber: That too.
Meredith: (Yelling) Who's on my service?!
Amber: (Rolling her eyes) Jo's handling it Meredith! She's got this! (Turns to Andrew) Doctors are the worst patients I swear.
Andrew: I can't even imagine how you would be in a patient bed.
Amber: (Chuckles darkly) I think half the staff would quit after a day with me.
(Teddy comes out of the room with a vial of blood)
Teddy: Okay Karev get this up to path tell them to check the inflammatory markers and D-dimers, put a rush on it. If they give you grief tell them it came from me I have another patient, get back to me with the results.
(Amber nods and leaves to go to path when she gets a call and answers it knowing who it is)
Amber: I'm headed to path with her blood right now, we're gonna look at her inflammatory markers and D-dimers. Her pulse ox is still at 92% and no fever in the last 48 hours any questions?
Alex: Hey Amber it's nice to talk to you too. How did her scans look?
Amber: She's uh she's refusing them actually and all other tests.
Alex: What? Are you serious?
Amber: Yeah, I know, I know but it came from her she thinks she's wasting our resources.
Alex: She's not, why the hell would she think that?
Amber: Our beds are almost at capacity; our ventilator usages are through the roof and our CTs are overbooked. I'm not saying she's not worth it but she doesn't think her condition warrants concern.
Alex: (Groans) Damn it Mer, I wish I was there to knock some sense into her.
Amber: (Sighs and feels bad for Alex) I can talk to Altman and get her to run a CT against Mer's advice. It's not the same but a Karev looking out for her is kind of like you being here.
Alex: No, I don't want you to get into trouble.
Amber: (Snorts) You and Meredith conspired to commit insurance fraud and you're still practicing. I think I'll be okay plus it's not exactly the right time for a medical trial, I'll talk to Altman and call you with the results.
Alex: Get Altman to check her CRP, ESR and D-dimers again, we need to cover all the bases.
Amber: Okay I will, bye.
(Amber hangs up as she heads to the elevator)
LATER AT THE COVID WARD
(Amber approaches Teddy with the results)
Amber: Her inflammatory markers and D-dimers all came back within normal range.
Teddy: That's good, lets check on her and see if she's stable enough for discharge.
Amber: Dr. Altman with respect I don't think that's a good idea. I mean we've seen how these patients are at first, they're fine and then they take a quick turn. I would get another CRP, ESR, D-dimer, and I'd like to schedule her for a chest CT before we even consider discharging her.
Teddy: I'm sorry, "I"?
Amber: We. You. Whomever. The point is it is too soon to discharge her.
Maggie: What's going on?
Teddy: Meredith wants to leave to go to a hotel. Her pulse ox is above 92%, her ABG looks good, no fever for 48 hours. I'll check her daily labs, but she does not want any more tests. And this is protocol. And she wants us to follow it.
(Teddy leaves Maggie and Amber out in the hall)
Amber: (Scoffs) Grey breaks protocol for every patient. We can't break it for her?
Maggie: (Not satisfied) Part of being an attending means no longer asking for permission. I'll get her to reconsider, come on.
(Maggie and Amber walk to Meredith's room where they find her bed empty. They quickly go inside and find her on the floor passed out. They immediately go to her)
Amber: Dr. Grey?
Maggie: (Panicking) Meredith! Oh my god.
Amber: Hey Meredith can you hear me?! Let's turn her over.
(They turn Meredith over and put the oxygen mask on her)
CUT TO BLACK
SCENE: MEREDITHS ROOM
(Amber checks Meredith's sats while Maggie tends to her)
Maggie: Mer?
Amber: Meredith?
Maggie: Hey, Mer, you passed out.
Amber: Damn it, sats are 88.
Maggie: Hey, do you know where you are? Can you hear me?
Meredith: (Tired) I can hear you. And you're loud.
Maggie: Well, sorry. Come on. Take a deep breath for me. Come on.
Meredith: (Groans and coughs) I just was trying to take a couple of steps. I thought I could make it. I want to go to the beach.
Amber: The only place you're going is radiology for a chest CT, Dr. Grey.
Meredith: No more tests.
Maggie: Meredith, that is where we are going right now.
(Amber and Maggie wheel Meredith out of the room)
LATER AT RADIOLOGY
(Amber is on the phone with Alex outside the screening room giving him an update)
Alex: She passed out in her room?!
Amber: She said she wanted to try walking.
Alex: Well, what the hell happened?!
Amber: I-I don't know Alex she was fine an hour ago and then her sats dropped to 88. I thought she was out of the woods but we're getting a chest CT right now.
Alex: You need to get her treated right now.
Amber: How? We don't know anything about the virus, we don't have any treatments except O2 and rest, there's no protocol for this Alex.
Andrew: Yes, there is.
(Amber turns to see Andrew behind her)
Amber: What?
Alex: Who's that?
Amber: DeLuca.
Andrew: Put him on speaker.
(Amber puts Alex on speaker)
Alex: What is it?
Andrew: I've got a dozen ICU patients on interferon alpha 2b, anti-retrovirals, antimalarial drugs, it's a new protocol that I'm trying.
Alex: And your seeing positive results?
Andrew: With some yes.
Alex: Okay get her on this protocol.
Amber: Alex we don't know if we're there yet we need to see her scans first and then we come up with a plan.
Alex: Then what the hell are you doing talking to me? Go and see how she is now!
(Alex hangs up and Amber grips her phone in frustration)
Amber: Let's go, you're coming with me, you seem to know more about this than I do.
(Amber and Andrew go inside the screening room with Teddy and Maggie already inside)
Teddy: What's DeLuca doing here?
Amber: We need all the help we can get and he has more hours in the covid ward than me.
Maggie: Okay he can stay; we're waiting for scans now.
Amber: (Looks at Meredith inside) We should have gotten scans hours ago.
Teddy: She felt fine hours ago.
Amber: You know as well as I do how this thing usually goes. They think that they're on the other side of this thing, and then they tank.
Maggie: Karev-
Andrew: She's right instead of waiting around, we should have started interferon alpha b, anti-retrovirals, antimalarial drugs. I got a-a dozen patients in the ICU already on this protocol.
Maggie: And you're seeing positive results?
Andrew: With some, but I've also read a lot of studies that show that these meds, they reduce the sev-
Teddy: What studies?
Maggie: Are these peer-reviewed?
Teddy: Have they been published?
Andrew: They're credible.
Teddy: Nothing is credible. Not yet. And I don't feel comfortable giving Meredith Grey some experimental drug that's gotten some foot traffic on a Reddit board.
Amber: (Sighs in despair)
Maggie: Let's just get the scans first. We can fight about the plan later.
(They stand by and wait for the scans)
LATER AT THE PIT
(Jo goes inside to her consult)
Jo: Val Ashton? Hi. I'm Dr. Karev. Okay, so you've had abdominal pain and vomiting for the last two weeks?
Val: But it's not Covid. I have gone nowhere, seen no one, touched nothing. I couldn't stop talking to the nurse that took my blood. It was the first in-person conversation I had had in months. I told her about the fight with my co-worker, all about my ex. And I think I invited her to lunch.
Jo: (Chuckling) Don't worry. They're used to it. Okay, so your labs came back already, and…congratulations. You don't have Covid. You're just pregnant.
Val: (Shocked) What?
Jo: Pregnant.
Val: (Still shocked) What?
Jo: Yaay?
LATER AT THE COVID WARD
(Amber and Andrew approach Teddy who paged the both of them)
Andrew: You paged the both of us?
Teddy: Yes, I did, as you both saw from the scans Meredith has developed ground glass opacities of the lungs since yesterday that means she's gonna get worse.
Andrew: Yeah, if there's anything I can do just let me know.
Teddy: Actually, there is something the both of you can do. Maggie has requested that the both of you be on her medical team, nobody has seen more covid patients than either of you and I agree with her suggestion. Karev, you have to monitor outside not inside, got it.
Amber: Yeah, I'll keep my distance and see if there's any new protocols or treatments that can help speed up Grey's recovery.
Andrew: I'll send you a piece of the protocol I'm trying and see if Meredith's a good match.
Teddy: Thank you but I need to ask the two of you this for the sake of Mer. Are you both good to work together? I know there's personal history with you two and I know how it can affect the professional but now isn't the time for that. Meredith Grey needs our full attention; we all know what happens when a patients lungs start to look like that. They could turn on us at any moment so I need you two to be sure that you can work together effectively and not let it disrupt what we have to do to save Meredith Grey, can you do that?
Amber: Yes of course.
Andrew: Yes.
Teddy: Good, Karev, I want you to monitor Grey from outside don't let her out of your sight and page me if her sats begin to drop.
Amber: Okay I'll watch her like a hawk.
Teddy: DeLuca, I want you to search up any protocols and treatments that are somewhat credible and can help Meredith.
Andrew: I already got half a dozen on file if you want to look it over.
Teddy: Send it to my email. Now let's get to work.
(Teddy leaves them both. Amber grabs a chair and sits in front of Meredith's room keeping a close eye on her monitor. Andrew notices the sad look on her face)
Andrew: Are you okay?
Amber: (Composes herself) Yeah, I'm fine, you can go I've got this.
(Andrew doesn't believe her but leaves her to it)
LATER AT RADIOLOGY
(Jo and Levi stand by as they wait for Val's abdominal scans. Jo groans and rubs her temples)
Levi: Are you okay?
Jo: I'm fine it's just a headache, I guess taking on Grey's service has gotten to my head.
(Bailey appears to them)
Bailey: Karev, I need you to keep covering Grey's service. I'm hoping to persuade Dr. Webber to come back to the OR, whenever he finishes adjusting his crown. Chief of chiefs. I could be chief of chiefs.
Levi: You'd be really good at that.
Bailey: Hmm. What do you got?
Jo: Abdominal pain, vomiting, positive beta HCG. She's not pregnant, but I want to rule out a mass.
Levi: Mass! There. In the liver.
Bailey: (Intrigued) That is no mass. That is a fetus.
Jo: (Shocked) She is pregnant, but her baby is attached to her liver?
Bailey: (To Levi) Mine.
Levi: Uh, th-this patient doesn't have Covid. I'm allowed to be on the case.
(Bailey glares at him and Levi caves and leaves the room)
Bailey: (Happy) I feel better.
LATER AT THE COVID WARD
(Jo finds Amber in front of Meredith's room watching the monitor)
Jo: Hey I heard you were put on Mer's team.
Amber: Yeah, Maggie handpicked me I wish I could feel flattered. What's up?
Jo: …We need to talk about Alex.
Amber: I know he's worried about her but we all are.
Jo: Yes, but one of us is her POA and is currently in another state right now. Amber, you know that Meredith needs someone local to make this decision for her and it can't be Alex.
Amber: What do you want me to do about it? Meredith gets to decide who is in charge of her not Alex. Do you want to convince her?
Jo: God no, Bailey is gonna do that, but I'm hoping that you can inform Alex and he can go along with this accordingly.
Amber: (Sighs) I'll talk to him if Meredith changes her mind, I'm sure he'll understand but we need to get there first. Let's talk about something else please? I am getting antsy staring at this monitor.
Jo: (Grins) I am about to perform my first post covid surgery. It's an abdominal pregnancy of a 26-week-old fetus attached to the liver.
Amber: (Shocked and looks at Jo) No! alive?
Jo: (Nods in joy)
Amber: (Jealous) Ugh, I hate you so much.
Jo: (Chuckles) Yeah, I hate me so much too. I'll see you later I have to scrub in.
(Jo leaves Amber to stare at the monitor again)
LATER AT O.R. 4
(Hayes gowns up as Jo and Bailey work to get the baby out with Carina on standby)
Hayes: Someone's messing with me, right? We got a 26-weeker attached to the liver? Still viable?
Bailey: Come see for yourself. Liver's exposed.
(Hayes and Carina look at the fetus in amazement)
Carina: Wow.
Hayes: Hello there, little one.
Bailey: Okay, let's go through it again.
Jo: The placenta has left the liver extremely friable. We cut. We get the baby out. We leave the placenta intact. Otherwise, she could bleed out.
Bailey: Which she will not. Everyone in this room needs a little joy. Ready?
Hayes: Ready.
Carina: Weirdest C-section I've ever done. Also ready.
Bailey: Here we go.
Carina: Cutting into the amniotic sac.
(Carina cuts the amniotic sac and gets the baby out)
Jo: Clamps for the umbilical cord.
Bailey: Heavy scissors. Thanks.
(They cut the umbilical cord on the inactive baby)
Carina: I'm not seeing any movement.
Hayes: Prepare to intubate immediately.
Jo: (Works on Val with Bailey) Where is all this blood coming from?
Bailey: Oh, the right hepatic artery. More suction.
Jo: How's Luna?
Carina: Apgar 2, maybe 3.
Hayes: Still trying to get the tube in. Come on. Come on, come on, come-I'm in. Bag her, please. Okay, we need to get her up to the unit as fast as possible. Go. (Puts Luna inside incubator) Ready, let's go, let's go, let's go, come on.
Bailey: More lap pads, more. More, more-
Jo: We should resect the right lobe.
Bailey: No, that could lead to liver failure. No.
Jo: Then we put her on dialysis and get her on the transplant list.
Bailey: That's not as easy as it used to be. Hold on.
Jo: I'm not gonna let her die before she sees her kid. Clamp.
Bailey: Hold on! Hold on!
(Jo freezes with Bailey thinking for a moment while Val continues to bottom out)
Jo: Bailey?
Bailey: Okay. Okay, do it.
Jo: Bovie.
(They both work to save Val)
MEANWHILE OUTSIDE MEREDITHS ROOM
(Meredith gets a call from Amber on her tablet. She answers it while Amber is right outside the window communicating through facetime)
Meredith: You look like hell.
Amber: Yeah well, I feel like it too not as much as you though. I take it Bailey talked to you about changing POA's.
Meredith: Is this you convincing me not to?
Amber: God no, Alex is more of an earful through the phone than in real life. I can't imagine how it would be for the next two weeks. Plus life and death decisions should be made by someone within a 5 mile radius from you or maybe 5 feet if your interested.
Meredith: Is this you volunteering as my Alex?
Amber: (Shrugs) Why not? I'm a Karev, aren't I? I'm young but I've seen some pretty horrific stuff that trumps pulling the plug on my brothers best friend. I'm pretty transverse in deaths during a pandemic and I couldn't care less about you and your kids. I think I'm pretty good on paper.
Meredith: Thanks, but I'll pass, I'm not gonna let Alex resent his only sister for pulling the plug even if I wanted her to. God knows he needs you in case this gets worse.
Amber: I had to try…I was always a little jealous of you, you know.
Meredith: Because I'm an award-winning surgeon?
Amber: Because you got my brother when I didn't. I…I know the bond you and Alex have and I'm grateful he had someone there for him during all his craziness but…but I had no one when he had you. He held my hand for ten minutes after I was attacked but he never left your side even from a thousand miles away. He has called about you more in the last eight hours than he's called about me in eight years. I'm jealous you got Alex's brotherly side to protect you and I didn't. I guess what I'm saying is try to fight a little so he can get the sister he chose back into his life when he gets here. I know him well enough to know he would be devastated if you died without him by your side. So please fight, okay?
Meredith: I'll try…and for the record he would be just as devastated if you were me, I know him well enough too.
Amber: Yeah, we'll see when that day comes, your sats look stable, find someone to kill you who is within walking distance.
(Amber hangs up)
Andrew: How are you holding up?
(Amber turns to see Andrew behind her)
Amber: (Sighs) How much of that did you hear?
Andrew: All of it, how's she holding up?
Amber: Her sats are still the same but she's getting more tired so I think this is just the beginning stages of it getting really bad. Have you found anymore treatments on Reddit?
Andrew: No not one's that are peer reviewed but it's hard to find that for a new virus. Are you sure your okay to do this?
Amber: Andrew we're friends not close friends but at least we're not sniping at each other. I call that a good relationship for the work we're about to do.
Andrew: No that's not what I mean, I mean are you okay to work on Meredith Grey? Because she's not another patient she's…she's your brother's best friend. I'm asking if you feel up to treating her and making sure you keep your sanity intact.
Amber: (Exhales) I'm nervous but I'll be okay. I know that without Alex here she needs a Karev to watch out for her and lucky for us both I am it. I am in control Andrew I am aware of the risks that come with this but I don't care. I may have been jealous of her but I am also appreciative of how she put up with Alex at his absolute worst and brought out the best in him. And as a result, getting my brother back. This is the least I can do for her so I am good to be on her team, are you?
Andrew: Yeah, I am. You should go home though you need rest before tomorrow.
Amber: I can stay.
Andrew: Amber you're watching a monitor which is something even the new interns can do. I already have Helm on the night shift taking monitor duty which she was happy to take. I know you want to help but as someone once told me you can't treat someone if you're exhausted.
Amber: You're quoting me to me, really?
Andrew: You give good advice sometimes you just have trouble following it so please go home and rest, your gonna need it.
Amber: (Sighs) I hate it when your right.
(Andrew grins as Amber stands up and leaves)
MEANWHILE AT THE ATTENDINGS LOUNGE
(Jo puts on her casual clothes exhaustingly and looks at her calendar on her phone. She freezes when she sees the date and calculates in her mind before her eyes widen in shock)
LATER AT ALEX AND JOS HOUSE
(Amber comes up from her basement having just taken a shower and finds Jo sitting on the couch with a blank face)
Amber: Hey when did you get here?
Jo: Just a little while ago, you were in the shower when I came in.
Amber: Well, I am glad you are here that means I won't have to drink alone. It's probably my last free night in a while so I want to make the most of it. Plus, you just did a surgery on an abdominal pregnancy so I think it also calls for a celebration. (Looks at the fridge) What do you want? Wine? Martini? Maybe both with a Prozac back? It's your choice so what's our poison tonight?
Jo: (Calm voice) None I don't want to drink.
Amber: (Confused) What? You just did your first surgery in a month and you're not drinking? Or did you already drink? Are you drunk?
Jo: What? No, I'm not drunk I'm…hungry, I guess.
(Amber scoffs in disbelief and pours herself a glass of wine before walking toward Jo on the couch still frozen)
Amber: (Makes light) Okay you just extracted a baby from a liver and you don't want to celebrate and drink? What are you pregnant or something?
(Amber chuckles lightly as Jo looks at Amber calmly. Amber stops chuckling and her eyes widen in shock)
Amber: (Shocked) What?
(Jo says nothing as she looks at Amber who sits on the couch)
Amber: Jo? Are you…?
Jo: (Grins slightly) Um…according to the pregnancy test I took while you were in the shower…yeah, I think I am.
