John Doe woke up at the Blackfield Asylum,a mental institution. He had suffer amnesia and couldn't remember what happen. He and the other patients met Calypso,who runs the Twisted Metal contest and will grant them wishes if one will win.

He offers the patients Vehicles with Guns.
Before they get ready,The Patients tells Calypso what happened. John tried to remember,but he couldn't. He earns a Vehicle called Roadkill.

During the First Round,No-Face was killed by Needles Kane with his vehicle,Sweet Tooth while Raven and Dollface were spared. John Doe begin to remember:He was in the bank where a man throws a grenade before he gets knocked out.

Before the second round,John was confronted by Agent Stone,who reveals that he and John were both FBI Agents before he ruined everything. John suddenly remembers what had happened:
During the robbery,the woman held hostage and Stone disobeys the hold before John stops him. The hostage free herself and the robber shot Stone's hand before getting himself shot as well. The other robber throws a grenade,killing two FBI Agents and knocking out John as well.

During the second round,John managed to damage Stone's vehicle and kills him. received a Monster Truck,Slayer and kills Bloody Mary,Billy Ray Stillwell,and Preacher. John Doe outsmarts ,killing him.

Before the Third and final Round begins,Calypso tells John Doe all his memories. He has a family. During the final round,Needles will be keeping on attempting to kill him. John manage to escape by destroying the weakened wall.
He drove his car out of it and into the ocean.

Needles and Calypso begin to believe John has committed suicide. Needles,whose real name is Marcus,approaches Calypso,having won the contest. He gave Needles a vial that contains the forget me serum. Calypso tells him that if he drinks it,Needles will forget that he is a serial killer. Needles choose to stay remembering and kills Calypso. Needles kills all the guards and escape.

John's family went to the farmhouse where John appeared,having faked his death to protect himself and his family.
