My friend loves his tea and I'll admit it super matches his whole vibe. Top hats and tea cups are like cola and peanuts, you know what I mean? Plus, he's always drinking milk because he's super old and his bones need it.
But it all just goes straight through him. He has no bladder!
Every single meal, all of us have to sit around the table and pretend we don't hear the drip, drip, dripping of liquids splashing on the floor. I don't wear shoes! Neither does Chopper! It's super gross, yo!
Plus, everyone has to pretend they don't notice his soggy bottom when he gets up and walks around.
Huh. I wonder if he's always asking about other people's underwear to see if he's the only one packing a wet pair?
Anyway, would I be the super asshole or the good friend to ask him to wear Attends?

NTA. This sounds like a very uncomfortable situation for everyone involved. Incontinence is a common problem for the elderly, but it doesn't make it any less embarrassing or difficult to confront. You sound empathetic and frustrated, but NTA.
I think you should be straight up with your friend. He'll be more comfortable in dry clothes. (And top hats do not go with soggy bottoms!)
You may also want to have him check in with his primary care provider. If he's asking other people inappropriate questions about their underwear, that could be a sign of dementia.
You were vague on that point, but it's hard to imagine underwear appropriate questions to ask anyone.
