The next afternoon Owen was paged to the OR to find Teddy on the ground. She hadn't been feeling well and was complaining of a tooth ache, he had just booked the dentist appointment she was too stubborn to book for herself.
"Page dr Deluca," Owen commanded, seeing Teddy down on the floor.
"Dr Deluca," Mika questioned.
"Get a gurney and get her off the floor," Owen roared.
Owen wondered if it was a ruptured ectopic pregnancy, with Teddys age and medical history it could easily be that or a blood clot.
"Deluca," Mika repeated.
"Deluca and Roberts," Owen barked, Roberts was young, just past her fellowship but she was cardio and would have to do it until someone more experienced arrived.
At the same time Christina Yang walked though the front doors she hadn't passed through in a decade. She looked around noticing the changes that had been made since she left.
"Dr Hunt," Mika asked.
"45 year old female, 7 weeks pregnant, history of blood clots in pregnancy," Owen recited, "blood type O+, not on any medications, shotgun her,"
"Dr Hunt," Mika replied.
"Treat my wife," Owen roared, "page everyone."
"Dr Hunt, we need help," Lucas said.
"I've paged cardio," Simone replied.
They got Teddy onto a gurney and into the next OR, they could do all of the tests there without bringing her down to the ER then back up.
"Dr Hunt," Carina yelled, "out of the OR."
"Teddy," Owen replied, "I have to be with my wife."
"Then you stay out of the way," Carina replied.
Carina grabbed the portable ultrasound she had asked for and began scanning Teddy's lower abdomen, "it's not ectopic, there is an embryo in her uterus. Good heart beat, it's viable if she can be stabilised quickly. She's not bleeding in her pelvis."
Blood was taken and quickly sent to the lab, while Teddy was given IV fluids and medications to stabilise her blood pressure and heart rate.
Time seemed to crawl while they waited for tests to come back.
Roberts scrubbed into the OR and called for the portable echo.
"It's endocarditis," Roberts declared, "her heart is working hard but I'm not seeing major damage. There's some inflammation in the pericardium and around the valves but it's not surgical yet. It would be a greater risk to open her up."
"We had to shock her," Mika replied.
"She was in V Fib," Owen replied.
The phone in the OR rang, Roberts spoke to the lab and nodded.
"She has a massive bacterial infection," Roberts turned to the room ,"she's in septic shock. We need to take her up to ICU and pump her full of antibiotics and pressors."
"No surgery," Owen asked.
"Not until we find the source of the infection," Roberts replied, "then we remove it."
"She had a tooth ache," Owen replied.
"It likely abscessed," Roberts replied.
"Keep her her," Owen demanded, "keep working on her and get an oral surgeon in."
"We don't have," a nurse said.
"Get one," Owen roared, "get one here now."
"Dr Hunt," Carina tried putting a hand on his arm.
Owen shook her off, "get an oral surgeon here NOW!"
They started paging the few on call oral surgeons they had hoping someone, anyone would come.
"Owen you need to step out of this OR," Carina said, "Dr Wilson is a general surgeon she is going to take over and keep Teddy stable until the oral surgeon arrives."
"Let's go," Link barked, grabbing Owen by the shoulders and dragging him out of the OR.
Christina heard the commotion and headed up to the OR floor to see if she could help.
"Let me," Christina offered, approaching the scrub room where Link had dragged Owen.
"It's Teddy," Owen yelled, "they have a brand new surgeon working on Teddy."
"Get me scrubs," Christina barked.
"Christina," Owen asked.
"Who are you," Link asked.
"Dr Christina Yang," Christina introduced herself, "director of the Klausman institute. Teddy was my teacher and I'm Owen's ex-wife."
"Let her," Owen said, "it's Teddy. She has to be okay."
"Owen I don't know her history," Christina said, "I know she was healthy when we worked together but."
"She's 7 weeks pregnant, her second pregnancy, she had a blood clot with Allison, our daughter at the end of the first trimester," Owen replied, "cervical insufficiency around 32 weeks. She's Teddy. She's still running almost daily in the gym 3-4 days a week playing pretty physically with the kids. She's the chief. And has had a tooth ache for 2 or 3 days. They've called an oral surgeon."
"Dental infection leading to endocarditis," Christina asked.
"Possibly," Link replied, "I have to go with Jo Wilson and try to save our patient. Hunt is going to stay here. Can you?"
"I will," Christina replied.
Christina quickly changed and scrubbed in.
"Teddy," Christina said, "you taught me to do this now I need to operate on you and save you and your baby. You did the one thing no one else could for Owen. You gave him a child of his own."
"Dr," Mika asked.
"Dr Yang," Christina replied looking at the intern, "you are?"
"Mika Yasuda, intern," Mika replied.
"She was my teacher 10 years ago," Christina said, "and she's yours now can you do this with me on your teacher?"
"I can," Mika replied.
"Okay it's simple," Christina replied, "we're going to drain the infection around her heart hopefully without having to fully crack her chest. Teddy can't do that healing time not with two preschoolers."
"She's," Mika started.
"I talked to Owen," Christina replied, "I did my residency here. I know both. I know there's lots of people here that love and respect her. I'm one of them. I wouldn't be the surgeon I am without Teddy."
Christina looked at the few images again and picked up the scalpel, she could do this as a key hole surgery. She didn't need to open up her mentor fully, this was doable as a keyhole procedure. Christina worked quickly telling Mika what to do and giving the same precise type of instructions Teddy used to give her while letting the young intern try as much as possible. Satisfied Christina closed up and stood guard while the oral surgeon found the abscessed tooth and performed the root canal to save the tooth. Teddy would need to see her dentist for a crown when she was discharged but she would heal. Carina kept watch on the tiny embryo.
"The baby," Christina asked, knowing how much both Owen and Teddy loved and wanted children.
"Stable heart rate," Carina confirmed, "it's early and time will tell."
"Close and take her to the ICU," Charistina commanded, "Yasuda you're with her until another intern replaces you. She's not to be alone."
Christina dictated her last orders and scrubbed out.
Owen sat anxiously in the locker room outside the scrub rooms, he looked up hearing the door bang open.
"Teddy," Owen gasped.
Christina took a seat beside her ex husband on the bench.
"Is going to ICU," Christina replied, "she will make it. Your foetus is fine but it'll be a long road Owen. The foetus may not be viable with all of the medications and sedation we've given Teddy. I had to save her first. You can try again."
"She's nearly 46," Owen said, "the baby was a surprise, only the two of us know and Teddy wouldn't survive losing a pregnancy."
"Owen we had to save her life first," Christina replied, "your other kids wouldn't survive losing their mom. We both lost our dads young and know how hard that was, think about your kids losing their mom or you. Teddy will be okay. She's strong and generally healthy."
"What did you do," Owen asked looking at Christina for the first time.
"I did a keyhole incision for pericardial wash out and removed the infected fluid, the valves look good," Christina replied, "I was able to get a scope in through her groin, she'll need to be rechecked in a couple days and monitored for a few months but Teddy will be okay. She'll be okay Owen. it'll be a long road and hard for all of you but she's alive and will recover."
"I can't lose her," Owen gulped putting his head in his hands.
"You won't," Christina promised, "you have me as her surgeon. She's my patient. I care about both of you."
"It's Teddy," Owen said, "it's Teddy. I can't lose Teddy. She's always been there, been here, the worst days of my life Teddy was there."
"You won't," Christina assured, "she should be up in ICU now and you can stay with her."
"It's my fault," Owen said, "I didn't push her to see the dentist, I should have scheduled one for her or made her go when she took the kids a couple days ago."
"It's no one's fault," Christina replied, "it's not your fault or anyone else's."
"But," Owen started.
"Go be with your wife," Christina replied, "I'm here to teach and need to check in with the intern."
Owen made his way up to the ICU and found Teddy. He sat by her bed holding her hand feeling defeated, she was Teddy, this was his best friend, his person, the person who always came through for him no matter how badly he messed up and he had failed her. He had missed this and let her get hurt and sick. He had let her down, part of her taking over as chief was that he was picking up more of the household stuff day to day, groceries and cooking, house work, kids schedules while she got settled. He had been so focused on the kids and helping them adapt to mom's new job he hadn't been taking care of Teddy.
Teddy lay still and pale in the bed, her scrub cap still covering her hair. She was intubated but looking at the monitors Owen thought they could and should take the tube out soon even if she was sedated. He needed to call his mom and get her to come for the kids but he was frozen. He had to stay with Teddy and take care of her.
