Izuku Midoriya, a powerless child in a world full of people with superpowers, was experiencing the worst day of his life. One day, he ends up taking the wrong path, which leads to the life changing events that will turn him into something more, something that even the society full of superpowered humans can't comprehend.

Izuku Midoriya always wanted to be a hero. His quirklessness was the only obstacle, but when society deemed him useless and a waste of space, there is only so much a person could take. Feeling despair eat at him from the inside, Izuku didn't notice where he was when a rumble shoved him off the rooftop. He survived, but not in the way he expected. (I don't own Dark Souls, nor MHA)

Izuku Midoriya, hero hopeful, quirkless, and also a heavily abused teen, gets pulled into The Lands Between after dying to a villain when he tried to save his former tormentor. Ten months later while traversing Liurnia of the Lakes, Izuku is suddenly brought back to Japan, with no idea how he got there. Maybe this time, he could be a hero.

A lone Ghost is dropped into a world not their own as an experiment by the Vex. They swear to find a guardian that can destroy the vex once and for all. But first they have to survive this new world with allies and enemies around every corner. Oh, and Highschool is a battleground of its own. How will a guardian shape up to becoming a hero, and can this new world handle him?

Harry Potter packs all his stuff after the war, grabs Teddy, and tells Death to take him somewhere else. He doesn't expect to be taken to a world where powers are the norm! Not too long ago, I saw some fanart of a character swap between Marvel and MHA by DuckLordEthan. I’ve seen a few others put their spin on the idea in fanfics, possibly the best example was by Nerdmaster19. But he never finished his story, making me think about what might’ve been.
