Rick Taylor & his girlfriend,Jennifer Willis goes to the house where she will be having an interview with Doctor Henry West. Rick begun getting ready to propose Jennifer until the creatures appeared and mortally wounded him. As he lays down bleeding,Rick hears a voice of a mask he found during his rehearsing proposal. The Mask senses that Jennifer is kidnapped and tells Rick that it can heal him if he puts it on. Having no choice,Rick puts on the mask,transforming him into a hulking beast powered by blood.
Rick follows West & Jennifer through other dimensions & time periods and learns that West is planning to bring the Corrupted into this world by sacrificing Jennifer. West believes that the Corrupted is the only way to bring back his love,Leonora,who died in cholera. It is revealed that Leonora was previously resurrected,but was brought back as a demon. Rick encounters her in a wicker man cage as the townsfolk are turned into monsters by the Corrupted. Thinking she was Jennifer,Rick attempts to save Leonora,but was attacked by her demon form and he was force to kill her.
Rick manage to rescues Jennifer & thwarted West's plan by ripping his arm off. The Overlord,Leader of the Corrupted,emerges,summoned from killings Rick committed and constructed bodies of 10,000 monsters. Rick & the mask manage to kill it,but a stray spirit possesses Jennifer. Rick tries to pry it off,but the Mask refuses because of Jennifer's possession. It is turned out to be Leonora.
