John Doe woke up in the Asylum,suffering amnesia. Later,He & the patients met Calypso,the man who runs the Twisted Metal Contest. They both agree to enter and win a prize. John dreamed of the Bank Robbery and the explosion that knocks him out.
All the Patients had their Vehicles with automatic weapons,including Sweet Tooth for Needles Kane,a serial killer Clown. John has his vehicle,Roadkill. The First Round begins. Grimm,Dollface,& Raven are eliminated,non-fatally while Sweet Tooth kills No-Face. Agent Stone,who is driving Outlaw,was only targeting at John.
After Round 1 ended,Stone reveals to John that he was his partner until he got them both in prison. John begun to remember: He & Stone are FBI Agents,who tried to stop Terrorists at the bank. Stone disobeys a direct order and almost got the hostage shot. 2 Terrorists are killed,but one throws a grenade. The blast has knocked John out. He then looks at the picture of his family and remembers. Preacher,who is driving Brimstone,warns John that the Winner will be given a death wish.
The Second Round begins. John battles with Stone & kills him. The Warhawk was deployed to attack the drivers,killing Junkyard Dog,Preacher,& Bloody Mary. John manage to outsmart the Warhawk,destroying it. The Second Round has ended. Calypso tells John that he will never get out alive.
The Final Round begins. John & Sweet Tooth battle eachother. He tricks him into firing a rocket into the wall,making a hole. John drove Roadkill out of it & into the ocean. Calypso & Sweet Tooth believes that he committed suicide. Needles is revealed to be Marcus & he was given the forget me serum by Calypso. Needles chooses to stay remembering & kills him.
He got on his Ice Cream Truck,transforms it into a Robot,& kills the guards before escaping.
3 Days Later,John,who survives the fall,returns home where he reunites with his family.
